120 seconds is too long for controllers which can't be
setup during initialization, because this value is only
used for Admin commands so also rename as it is.

Now data structure spdk_nvmf_subsystem_pg_ns_info holds all the
reservation information from the associate namespace, so for the
IO processing routine we don't need to send a message to the
subsystem's thread to check the IO command is permited or not.

Each subsystem's poll group will have a copy of namespace's
reservation information, for those NVMe commands which may
change the reservation state, the commnad itself should be
returned after updating each subsystem poll group's
reservation state. Then it's safe to check the reservation
state in each poll group's thread.

Intermittent FTL test failure (ASAN) #717 reported an error, in
ftl_halt_poller() call, ftl_anm_unregister_device() will release
controller first, while in ftl_dev_free_sync() the IO queue pair
will be released again.

The function now has to be called before application
exit. At the moment it only frees the dynamically
allocated DPDK command line option strings - something
that was previously done from an atexit() callback -
but there's more to free there.
Note: the function descriptions were partially copied
from equivalent DPDK functions.

DPDK 17.11 is the oldest version still supported by DPDK,
so drop support for DPDKs older than that in SPDK. This
lets us remove a huge amount of ifdefs.

Currently if module claims a bdev in examine_config,
it cannot start asynchronously. This patch changes this
behaviour by calling examine_disk on modules which previously
claimed bdev trough examine_config.

spdk_dma_malloc() is not required here, as the bdev
struct is neither DMA-able nor shared between processes.
While here, also allocate g_null_read_buf using
spdk_zmalloc() instead of spdk_dma_zmalloc(),
as spdk_dma_*malloc() is about to be deprecated.

Historically, all memory returned from spdk_*malloc()
used to be physically contiguous, hence it could be
addressed by offsetting just a single physical address.
Since DPDK dynamic memory management came along, the
above is no longer true. Memory returned from spdk_*malloc()
doesn't have to be physically contiguous anymore. The
phys_addr returned from spdk_*malloc() only applies to
the beginning of the allocated buffer and user can't
possibly know how big that "beginning" is.
The phys_addr parameter in spdk_*malloc() is useless on
its own in most cases and only suggests that the returned
buffer is physically contiguous, which is wrong.
This information can be returned from spdk_vtophys(),
which is the only safe way to retrieve physical addresses.
That's why phys_addr param in spdk_*malloc() is now
deprecated.

Not doing so can cause us to hit asserts during the shutdown path. This
should fix an intermittent failure we are seeing on the test pool where
we hit the assert rdma_req->state != RDMA_REQUEST_STATE_FREE in
spdk_nvmf_rdma_request_process.
Note that this problem doesn't cause any data corruption when debug is
not enabled, it just causes us to probcess a subset of commands through
the state machine one extra time suring qpair shutdown.

First of all, this struct was used when stopping
a session and wasn't directly related to any vhost
device despite its name.
Second, the struct contained just a single poller.
Instead of renaming it, we remove it. We can use
that poller pointer directly.

Try to group data members that are used often into the
same cache lines. We still need to find more space in the second
cache line of spdk_nvme_pcie_qpair so that the important
parts of spdk_nvme_qpair fit.

For NVMe devices, in conjunction with the new batching options,
it can be advantageous to artificially delay between polling
for completions. Add an option to slow this rate down.

This was broken in commit 2725b5191e [1], which added
an extra #ifdef SPDK_CONFIG_VHOST_INTERNAL_LIB without
including config.h anywhere within the same file.
Currently the virtio library always behaves as if it was
compiled with upstream rte_vhost. Fix this by including
spdk/config.h where necessary.

When users call the RPC to add a NVMe controller, block NVMe driver
will create controller and block devices based on Namespaces, so
wrapper the two actions together with a intermediate function,
this will make the code looks more clean for the coming patch.

When we have a shared receive queue, the number of outstanding items
associated with a completion queue is deterministic, and limited by how
many RECVs we have total in the SRQ. So, we can set the total size of
the Completion queue at the beginning of time and never resize it.

Discovered in bdev compression test. Vol load was using the index
of the logical map to set the allocated chunk map bits as well as
retrieve the chunk. Exposed with overwrite situations. Also clarrified
one var name that I missed in previous patch.
Also added UT that fails without the fix, passes with the fix.

This practically reverts commit 2fe7aa5e4 [1].
The extra rte_bus was supposed to allow running SPDK
as a non-priviledged user by enabling RTE_IOVA_VA mode.
DPDK uses RTE_IOVA_PA by default - which means there are
physical addresses used as memseg IOVAs and hence the
root access is required to retrieve those physical
addresses. This patch was supposed to be paired with
a different DPDK patch of mine, but DPDK rejected that
one. Instead, in DPDK 18.11+ the user can force iova mode
by specifying --iova-mode=<mode> command line option,
where <mode> is either pa or va.
Either way, apparently there are cases where physical
address contiguity is required even without UIO (#707)
so let's revert this patch and consistently stick with
RTE_IOVA_PA.
SPDK requires some more effort to support running as
a non-priviledged user anyway.

Windows Virtio drivers use indirect descriptors without
negotiating their feature flag, which is explicitly
forbidden by the Virtio 1.0 spec. "(2.4.5.3.1 Driver
Requirements: Indirect Descriptors) The driver MUST NOT
set the VIRTQ_DESC_F_INDIRECT flag unless the
VIRTIO_F_INDIRECT_DESC feature was negotiated.".
Violating this rule doesn't cause any issues for SPDK
vhost, but triggers an assert, so we can only run Windows
VMs with non-debug SPDK builds.
This patch removes the assert and allows Windows VMs
to be run with debug versions of SPDK vhost.

Introduce EMPTY, PRESENT and REMOVED states for Vhost SCSI targets. This
does not introduce any functional changes but opens a way to stop using
both removed flag and spdk_scsi_dev pointer as indicator if device is
removed or not.

Make the vdev initialization happen before calling
any vdev related functions. This is mostly needed
for an upcomming patch where additional step is
required after initializing the vdev and before
starting rte vhost.
On the other hand, this patch also fixes a technically
possible scenario where rte vhost starts processing
vhost-user messages and calling our ops before the
related vdev was initialized.

rte_vhost requires all queues to be fully initialized
in order to start I/O processing. This behavior is not
compliant with the vhost-user specification and doesn't
work with QEMU 2.12+, which will only initialize 1 I/O
queue for the SeaBIOS boot. Theoretically, we should
start polling each virtqueue individually after
receiving its SET_VRING_KICK message, but rte_vhost is
not designed to poll individual queues. So we use
a workaround to detect when a vhost session could be
potentially at that SeaBIOS stage and we mark it to
start polling as soon as its first virtqueue gets
initialized. This doesn't hurt any non-QEMU vhost slaves
and allows QEMU 2.12+ to boot correctly. SET_FEATURES
could be sent at any time, but QEMU will send it at
least once on SeaBIOS initialization - whenever
powered-up or rebooted.
Vhost sessions are still mostly started/stopped from
within rte_vhost callbacks, but now there's additional
concept of "forced" polling, in which SPDK starts
sessions manually, while rte_vhost still thinks the
sessions are stopped. This can potentially lead to cases
where a session is "started" twice, or gets destroyed
while it's still being polled (by force). Those cases
also need to be handled within this patch.

Array channels in the subsystem's poll group are indexed by
nsid - 1, so rename the previous num_channels to num_ms
makes more sense. Also embed the channels into a namespace
data structure here, and this can be reused in the following
patch.

Depending on whether SRQ is enabled, resources may be allocated
to the rqpair or to the rpoller. Create a struct to hold these
pointers that can be used in both locations to avoid duplicated
code.

This patch adds support for many-to-one configuration
for OCF bdev. Meaning that one cache instance can
be used to cache multiple core devices.
Without this change, it is not possible
to attach core device to existing cache instance.

spdk_iscsi_conn_read_data was changed to use sock_readv
instead of sock_recv as a refactoring effort.
However this change caused about 3% performance degradation
for write I/O.
Hence this patch changes spdk_iscsi_conn_read_data to use
sock_recv and changes spdk_iscsi_conn_readv_data to call
spdk_iscsi_conn_read_data if iovcnt is 1.

spdk_iscsi_get_dif_ctx() didn't work correctly for Data In PDU
because we had to get iSCSI task from PDU directly for Data In PDU,
and lun_id was not copied in spdk_iscsi_task_get().
This patch fixes these bugs and the DIF strip feature was verified
after applying this patch.

Users may want to connect specified controller at running time,
so this API will connect to the controller and return probe context
to users, users must call spdk_nvme_probe_poll_async() to initialize
the controller to the READY state before using it.

VHOST_USER_SET_VRING_CALL invalidates the previous
file descriptor that SPDK may be using on a different
thread. The new descriptor is stored inside rte_vhost
internals and is queryable with rte_vhost APIs, but
those APIs are too expensive to be called every tick
or every time we need to use that fd. Hence, we will
now stop the entire vhost session before processing
SET_VRING_CALL msg and restart it right after. SPDK
will query the most recent call descriptor on session
start.
We do not necessarily have to stop the device - just
letting the session know that its callfd has changed
would be enough. That's an area for future optimization.

DPDK 19.05+ gives us an ability to pre or post-process
any single vhost-user message. The user can either perform
additional actions upon some generic events, or can
implement handling for brand new message types that
rte_vhost doesn't even know about.
In order to smoothly switch to the upstream rte_vhost
and drop our internal copy, we introduce an SPDK wrapper
function to register SPDK-specific message handlers. For
DPDK 19.05+ this will use the new rte_vhost API to
register those message handlers, and for older DPDKs
this function simply won't do anything - as w assume the
internal rte_copy already contains all the necessary
changes and does not need any "external" hooks.
For now we use the message handlers to stop the vhost
device and wait for any pending DMA ops before letting
rte_vhost to process the SET_MEM_TABLE message and unmap
the current shared memory.

Our internal rte_vhost lib requires SET_VRING_ADDR to
flush a pending SET_MEM_TABLE. On the other hand, the
upstream rte_vhost will invalidate the entire queue
upon receiving SET_VRING_ADDR message, so we mustn't
send it at runtime. Both behaviors are strictly
implementation specific, but this message isn't needed
by the spec, so send it only if vhost was compiled
with our internal lib.

Make struct vhost_user_msg match the format defined in
the vhost-user spec. The custom additions were inhereted
from DPDK, but SPDK never made use of them. Also replace
direct sizeof(*msg) usages with a handy macro that was
already there. Finally add a VHOST_USER prefix to the
VHOST_MEMORY_MAX_NREGIONS #define.

It's disabled by default, so no functionality is changed yet.
The intention is to use the upstream rte_vhost from DPDK,
which - starting from DPDK 19.05 - is finally capable of
running with storage device backends.
SPDK still requires a lot of changes in order to support
that upstream version, but the most fundamental change is
dropping vhost-nvme support. It'll remain usable only with
the internal rte_vhost copy and with the upstream rte_vhost
it simply won't be compiled. This allows us at least to
compile with that upstream rte_vhost, where we can pursue
adding the full integration.

Generate and insert DIF for write I/O by stream fashion in
spdk_iscsi_read_pdu().
Verify DIF for read I/O in spdk_iscsi_conn_write_pdu().
Verification can be done in spdk_iscsi_build_iovs(), but how many
writes to socket is not known beforehand, and same range may be
verified multiple times. So verify DIF before starting to write to
socket.

Add an new API spdk_dif_generate_stream() to insert DIF into
the metadata space of newly read data block by stream fashion.
This API is tested by replacing the previous DIF generation such
that DIF is generated once all data was read.

This patch leaves metadata space in data buffer for read or write I/O
to generate and verify DIF.
For write I/O,
- in spdk_iscsi_read_pdu(), leave metadata space for every block
by calling spdk_dif_set_md_interleave_iovs() and then
spdk_iscsi_conn_readv_data(), and remember this in PDU.
- in spdk_iscsi_op_scsi() and spdk_iscsi_op_data(), set not data
length but buffer length to length of an iovec. The reason is
that the actual data length sent to SCSI layer will be larger
than the data length set in PDU. SCSI task holds data length
as an independent member and this change doesn't loose any
information.
For read I/O, in spdk_iscsi_build_iovs(), leave metadata space for
every block by calling spdk_dif_set_md_interleave_iovs().
The reason why DIF context is saved in PDU is that DIF verification
will be done in spdk_iscsi_conn_write_pdu() and DIF context will be
used there first.

Increase the size of write (data out) buffers and immediate
data buffers in the iSCSI library to store DIF. Increase is the
amount necessary to store 16 byte metadata block to the block
formatted iwth 512 + 16. 512 + 16 is the current maximum ratio
of metadata per block.
Use the macro SPDK_BDEV_BUF_SIZE_WITH_MD defined in
include/spdk/bdev.h for this purpose.
Besides, change the name of a helper function from
spdk_get_immediate_data_buffer_size() to
spdk_get_max_immediate_data_size() to differentiate the maximum
data size and the buffer size, and then to use the macro.

Increase the size of small and large buffers in the bdev layer
to store DIF. Increase is the amount necessary to store 16 byte
metadata to the block formatted with 512 + 16. 512 + 16 is the
current maximum ratio of metadata per block.
This change will be done in the iSCSI library in the next patch
and other libraries may be done later.
Hence add and use an new macro SPDK_BDEV_BUF_SIZE_WITH_MD(x) for
convenience.

Use new queue API to manage OCF queues dynamically
This change allows for dynamic creation and deletion of queues
on get_ and put_ io channel.
Qeueues no longer depend on number of cores in SPDK.
Queue-to-pollers mapping list is removed as well
as locks "q_ocf_lock" and "vbdev->_lock" as they became redundant

According to the implementation of VPP, the vppcom_session_write() may
cannot write all provided buffer, and we shouldn't continue in this
case, just like usage of vppcom_session_read().

The calling of spdk_bdev_finish is in wrong place,
the bdev subsystem recycling will be called by
_spdk_subsystem_fini_next. If we call spdk_bdev_finish
here, it will be called twice, so it is unnecessary.
And this patch will be used to address the issue

This patch extends existing ISA-L support for NVMe/SCSI T10 CRC-16
protection check (PI). The choice of which algorithm to use is set
via an existing configure parameter. The default for x86 systems with
appropriate CPU instructions is to us ISA-L.
Testing shows that PI checking/generation will run at device speed using
ISA-L. The table-based algorithm is much faster than the original algorithm
and is provided to speed up calculations on non-x86 systems.
This patch also fixes one issue in a false configure error if nasm
is at a higher version than currently checked (i.e. 2.13.03)
Local performance testing yeilds these results:
Original - 15 mbit/sec. (code removed)
Table - 400 mbit/sec. (27 times faster than original)
ISA-L - 3,400 mbit/sec. (227 times faster than original)

For API spdk_nvme_connect(), users can only use NULL or
struct spdk_nvme_ctrlr_opts for their own driver options,
so parameter opts_size doesn't take effect here. Since
some applications outside SPDK's git repository are using
such API, so we only remove the internal data structure
for now.

Ths patch adds an API spdk_iscsi_get_dif_ctx().
When opcode of BHS is SCSI command, spdk_iscsi_get_dif_ctx() extracts
CDB and LUN ID from BHS. When opcode of BHS is Data Out or Data In,
spdk_iscsi_get_dif_ctx() extracts TTT and get R2T task corresponding
to TTT, and gets CDB and LUN ID from the task.
Then spdk_iscsi_get_dif_ctx() gets LUN by using LUN ID and calls
spdk_scsi_lun_get_dif_ctx().
Additionally spdk_iscsi_get_dif_ctx() checks the state of the connection
and the type of the session is valid before starting decode operation
because contents of PDU is not reliable when the state of the connection
is invalid. The necessity of the check was found by the Calsoft test tool.
Checks and test cases are not matched yet but copying existing checks
from spdk_iscsi_conn_execute, spdk_iscsi_op_scsi, and spdk_iscsi_op_data
will be enough. The check added for Data Out is not necessary for Data In
but it will not be harmful for Data In.

This patch adds an API spdk_scsi_bdev_get_dif_ctx().
spdk_scsi_bdev_get_dif_ctx() decodes opcode in CDB, and if opcode
is read or write block commands, it gets LBA and use the lower
32bits of LBA as Reference Tag. It gets DIF information from
specified bdev next. Then it sets all to DIF context and return.
spdk_scsi_bdev_get_dif_ctx() is exported to iSCSI through
spdk_scsi_lun_get_dif_ctx().

iSCSI Initiator transfers, and size of metadata is not known until

This patch adds an API spdk_dif_set_md_interleave_iovs().
This function is used to leave a space for metadata for each block
when the network socket reads data, or to make the network socket
ignore a space for metadata for each block when the network socket
writes data. This function removes the necessity of data copy in
the SPDK application during DIF insertion and strip.

SPDK iSCSI and SCSI target don't expose any metadata and DIF settings
to the corresponding iSCSI and SCSI initiator.
Even when SPDK iSCSI and SCSI target allocate any bdev formatted
with DIF, SCSI commands sent from iSCSI and SCSI initiator don't
have any metadata and DIF information.
For that case, iSCSI target inserts and strips DIF on behalf of
iSCSI and SCSI initiator.
Hence SPDK SCSI target has to use data block size not including
metadata to process SCSI commands correctly.
This patch replaces spdk_bdev_get_block_size by
spdk_bdev_get_data_block_size in necessary places.

iSCSI target will need to get data block size except for metadata.
Other SPDK application may need to get the same information
future. Hence this patch adds an new API spdk_bdev_get_data_block_size
to bdev layer. In the header file, spdk_bdev_get_data_block_size
is located next to spdk_bdev_is_md_interleaved to avoid confusion
by new users.

Unregistering by a host may cause a reservation held by the host
to be released. If a host is the last remaining reservation holder
or is the only reservation holder, then the reservation is released
when the host unregisters. This may occur with Acquire/preempt
and Register/unregister commands.

A registrant can obtain a reservation on a namespace by executing
acquire command. Acquire command is associated with specific namespace.
For now only Acquire and Preempt reservation acquire action is
supported, Preempt And Abort will be supported in future.

Reservations can be used by two or more hosts to coordinate
acccess to a shared namespace, host must register to a namespace
prior to establishing a reservation. Unregistering by a host
may cause a reservation release, this feature will be supported
after reservation acquire patch.

Since the fuzz tester will be submitting random commands with random
memory addresses and such to the NVMe drives, we want to be especially
sure that we are using the IOMMU while running this test to prevent
memory corruption in the event that an errant command triggers a bad
DMA.
This function exposes to the application whether or not we are using the
IOMMU.

Not all RDMA drivers fail back the dummy recv and send operations that
we send to them when destroying a qpair. We still need to free the
resources from these qpairs to avoid eating up all of the system memory
after multiple connect and disconnect events. Since we won't be getting
any more completions, the best heuristic we can use is waiting a long
time and then freeing the resources.
qpair_fini is only called from the proper polling thread so we can safely
call process_pending to flush the qpair before closing it out.

We specify the --match-allocations rte init parameter
now, which gives us guarantees that memory will be
freed in the same units it was allocated.
Note that if user initialized DPDK separately from
SPDK, we aren't sure if --match-allocations was
specified, so will still mark the segments to not
be freed.

This function indicates whether DPDK was initialized
external to the SPDK env_dpdk library.
This can be used in cases where we need to implement
different behavior when DPDK is initialized outside
of SPDK - in that case certain flags that SPDK would
prefer may not have been specified. This will
be used in the next patch.

io_types like FLUSH and RESET are not always supported
by base bdev modules. For example: virtio_blk bdev doesn't
support FLUSH; ocf or ftl vbdev doesn't support RESET.

Upcoming patches will support DIF insert and strip feature in
iSCSI target and the feature will be implemented by utilizing
iovecs.
Even when we support the DIF feature, we want to keep current
batched PDU flush, and current requirement that there must be
enough free iovecs to map all segments of a PDU is too strict.
This patch alleviates the requirement by passing remaining
number of iovecs to spdk_iscsi_build_iovs().

Building iovecs had been done in spdk_iscsi_build_iovs() and
fast-forwarding iovecs for the partially written first PDU had
been done in-line separately.
This patch unifies these two operations into spdk_iscsi_build_iovs().
Fast-forwarding iovecs is necessary only for the first PDU, but the
operation is applied to all PDUs after this patch.
Extra overhead will be negligible because usually at most two
iovecs are consumed, one is for the base header segment and another
is for the data segment.

iovec_cnt and iovec_array are very descriptive and good but iovcnt
and iovs are often seen in SPDK and will be enough.
Subsequent patches will add some changes on iovec operations and
simple and familiar names will be helpful to work and review them.
This patch doesn't change any behavior.

This function add possibility to check if there are any scheduled operations
on particular thread.
Return from spdk_thread_poll() will be used as a way to load-balance and
signify if any work was performed during the single iteration.
A poller could return 0, but still be registered.
This helps especially in fio_plugin that only checked active_pollers or
messages via spdk_thread_poll().

This RPC was deprecated a couple of releases ago.
bdev modules now each have their own RPC for deleting
bdevs. Due to how bdevs are created differently on
different modules, it is simply not possible to
have one delete_bdev RPC that would work for all bdev
types.

We have conflict to handle the NVMf subsystem shut
down. The situation is that:
If there is shutdown request (e.g., ctrlr+c),
we may have subsystem finalization and subsystem
initialization conflict (e.g., have NVMf subsystem fini and
intialization together), we will have coredump
issue like #682.
If we interrupt the initialization of the subsystem,
following works should do:
1 Do not initilize the next subsystem.
2 Recycle the resources in each subsystem via the
spdk_subsystem_fini related function. And this patch will
do the general thing, but will not consider the detailed
interrupt policy in each subsystem.

Other io_type, like FLUSH, has a similar character with
UNMAP, that has a range description (offset and length),
but has no data payload. So the process for UNMAP io_type
can be extended to io_type like FLUSH.

When the decision was made to uncouple the number of shared buffers from
the queue depth and allow the user to decide for themselves, the default
was also significantly lowered, which caused some issues when trying
torun performance tests (See https://github.com/spdk/spdk/issues/699).
While this is a user modifiable variable, it is still best to keep the
higher default value.
The original value was equivalent to max_queue_depth *
SPDK_NVMF_MAX_SGL_ENTRIES * 2 with the defaults for max_queue depth and
max_sgl_entries being 128 and 16 respectively. Hence 4096

Default 'unmap' option stays as it was.
'Write_zeroes' comes useful when one wants to make sure
that data presented from lvol bdevs on initial creation presents 0's.
'None' will be used for performance tests,
when whole device is preconditioned before creating lvol store.
Instead of performing preconditioning on each lvol bdev after its creation.

When the specified buffer size to spdk_bdev_io_get_buf() is greater
than the permitted maximum, spdk_bdev_io_get_buf() asserts simply and
doesn't call the specified callback function.
SPDK SCSI library doesn't allocate read buffer and specifies
expected read buffer size, and expects that it is allocated by
spdk_bdev_io_get_buf().
Bdev perf tool also doesn't allocate read buffer and specifies
expected read buffer size, and expects that it is allocated by
spdk_bdev_io_get_buf().
When we support DIF insert and strip in iSCSI target, the read
buffer size iSCSI initiator requests and the read buffer size iSCSI target
requests will become different.
Even after that, iSCSI initiator and iSCSI target will negotiate correctly
not to cause buffer overflow in spdk_bdev_io_get_buf(), but if iSCSI
initiator ignores the result of negotiation, iSCSI initiator can request
read buffer size larger than the permitted maximum, and can cause
failure in iSCSI target. This is very flagile and should be avoided.
This patch do the following
- Add the completion status of spdk_bdev_io_get_buf() to
spdk_bdev_io_get_buf_cb(),
- spdk_bdev_io_get_buf() calls spdk_bdev_io_get_buf_cb() by setting
success to false, and return.
- spdk_bdev_io_get_buf_cb() in each bdev module calls assert if success
is false.
Subsequent patches will process the case that success is false
in spdk_bdev_io_get_buf_cb().

VMWare Workstation NVMe emulation does not seem to write the
SHST_COMPLETE bit within 10 seconds, resulting in an ERRLOG
during detach/shutdown. So add a quirk to cover these VMWare
SSDs. But rather than squashing the ERRLOG completely for
these SSDs, just add a message instead indicating this is
somewhat expected on these VMWare emulated SSDs.
Fixes issue #676.

Adapted our custom rte_vhost APIs to the upstream DPDK
version which has independently added similar APIs.
This will potentially allow us to remove our internal
rte_vhost copy.
rte_vhost_set_vhost_vring_last_idx() was renamed to
rte_vhost_set_vring_base() and the last vring indices
have to be acquired with a newly introduced rte_vhost_get_vring_base()
rather than rte_vhost_get_vhost_vring().
This is only a refactor, no functionality is changed.

Removing band from "free list" is moved from FTL_BAND_STATE_OPENING
to FTL_BAND_STATE_PREP state's change actions.
This will fix race condition when one band is prepared (erased)
and write pointer is trying to get next active band.

When an operation fails, we shouldn't pass a handle or
a 'valid' blob ID to the caller's completion function.
The caller *should* ignore it when bserrno != 0, but
it's best to not take that chance.
Fixes#685.
Note: #685 seems to have a broader issue related to
a possibly locked NVMe SSD in the submitter's system.
This only fixes the assert() that was hit.

There are some cases that virtual bdev open and close
the device and QoS will be disabled at the last close.
In this case, when a new bdev open operation comes again,
the QoS needs to be enabled again.

For real PCIe drives, if we removed one drive, existing hotplug
monitor will trigger the remove callback twice, there is one
workaround for vfio-attached device hot remove detection which
will also trigger the hot removal callback. For now we add
the check in the bdev_nvme layer so that coredump will not happen.
Fix issue #606.

It seems like DPDK 19.02 has split the "session mempool"
into two separate mempools but this isn't really described
in the DPDK release notes, so this patch only makes our
crypto code behave just like DPDK crypto examples.
rte_cryptodev_queue_pair_setup() no longer accepts
a separate mempool parameter but instead requires it
to be passed through a new field in struct
rte_cryptodev_qp_conf, which is also passed as a param
to rte_cryptodev_queue_pair_setup(). It's referred to as
"session private mempool" instead of "session mempool",
which makes some sense since we already use
rte_cryptodev_sym_get_private_session_size() (with the
word "private" in name) to calculate its size.
The other mempool - "session mempool" - now has to be
allocated with rte_cryptodev_sym_session_pool_create()
instead of regular rte_mempool_create().

We were only using one value from this array to tell us if the qpair was
idle or not. Remove this array and all of the functions that are no
longer needed after it is removed.
This series is aimed at reverting
fdec444aa8 which has been tied to
performance decreases on master.

Since we no longer rely on the state queues for draining qpairs, we can
get rid of most of them. We cn keep just a few, and since we don't ever
remove arbitrary elements, we can use stailqs to perform those
operations. Operations on Stailqs carry about half the overhead as
operations on tailqs

Allow user to add seed value for guard compuation to DIF context.
This will avoid the guard being zero in case of all zero data.
NVMe controller doesn't support seed value for guard computation
explicitly, and hence if we want to use such a seed value in
NVMe controller, we have to format metadata more than 8 byte,
and add seed value into the reserved metadata field.
But some popular iSCSI/FC HBAs and SAS controllers have supported
seed value for guard computation, and so supporting seed value
in the SPDK DIF library is very helpful for some use cases.
Hence this patch makes the DIF library possible to specify seed
value for those use cases.

Older versions of QEMU (<= 2.11) expose the VGA BIOS
hole (0xA0000-0xBFFFF) by specifying two separate memory
regions - one before and one after the hole. This results
in the "size" not being a 2MB multiple. But the underlying
memory is still mmaped at a 2MB multiple - so that's what
we should be checking to ensure the memory is hugepage backed.

spdk_env_opts->env_context may now contain a DPDK-specific
string that will be appended directly into rte_eal_init().
It can be used to e.g. override the default EAL loglevel,
which was hardcoded to RTE_LOG_NOTICE so far.
This is primarily meant to be used during development.
As a test for this feature, the vtophys test app will now
set the highest possible EAL loglevel which will give us
a ton of additional debug logs.
Note: the opts->env_context field is implementation-specific
and hence the vtophys app needs to check if it's run with
our env_dpdk. As SPDK_CONFIG_ENV is a raw text not even
surrounded with quotation marks, the vtophys app needs to
do a bit of #define magic to make it a string.

This patch expose backend's bdev's PI setting to the corresponding
NVMe-oF Initiator by Ideintify command, and removes the check if
block size is 512 multiple.
These change enables NVMe-oF Initiator to send extended LBA payload.

Move req->submit_tick assignments from specific transports to generic
qpair code.
Check whether submit_tick has been assigned before doing the actual
assignment, because a request may be submitted several times and the
original submit_tick shouldn't be covered.

In iSCSI, SPDK_ISCSI_MAX_SEND_DATA_SEGMENT_LENGTH was an alias
of SPDK_BDEV_LARGE_BUF_MAX_SIZE.
iSCSI had used both interchangeably.
SPDK_BDEV_LARGE_BUF_MAX_SIZE means the buffer size of the large
buffer pool in generic bdev layer, and will be changed to be
configurable.
SPDK_ISCSI_MAX_SEND_DATA_SEGMENT_LENGTH had been used to negotiate
MaxRecvDataSegmentLength with iSCSI initiator and to split large
read data, but both are determined by not iSCSI target but generic
bdev layer.
Hence this patch replaces SPDK_ISCSI_MAX_SEND_DATA_SEGMENT_LENGTH
by SPDK_BDEV_LARGE_BUF_MAX_SIZE.

This is in a effort to consolidate SCSI read and write I/O
for the upcoming transparent DIF support.
Previously conversion of bytes and blocks are done both in
SCSI layer and BDEV layer. After the patch series, conversion is
consolidated into SCSI layer.
About conversion from bytes to blocks, we don't expose bdev API
spdk_bdev_bytes_to_blocks and but create private helper function
_bytes_to_blocks because we will use not block size but data
block size when we support transparent DIF feature.

The last parameter xfer_len of spdk_bdev_scsi_read is not used,
and of spdk_bdev_scsi_write is used only to check task->transfer_len.
Hence remove the last parameter xfer_len from spdk_bdev_scsi_read/write
and extract the check operation from spdk_bdev_scsi_write and insert
it into spdk_bdev_scsi_read_write.
Additionally, remove a debug log because xfer_len is not passed to
spdk_bdev_scsi_write anymore. Hopufully, this will not degrade any
maintainability.
On top of this, factoring out the operation to convert byte to
block in spdk_bdev_scsi_read/write be done.

CPU profiling on workloads with intensive vtophys
operations (i.e. very small CB-DMA transfers) exposed
overhead introduce by spdk_vtophys having to call
spdk_mem_map_translate in a different compilation
unit. Let's just move the vtophys.c contents into
memory.c so that spdk_vtophys can inline
spdk_mem_map_translate and avoid this extra overhead.
This of course breaks the memory and vtophys unit
tests, so some additional changes are needed there
to keep everything linking correctly.

Previously, we can -p + hex value(e.g., 0x1) to assign the master core
and start the NVMe-oF or iSCSI target app.
However now it is not supported and prints error. I checked
the code, it only supports transformation with Decimal format,
so chaning the base to 0 to make it supporting other formats.

Add spdk_ioat_build_copy and spdk_ioat_build_fill
which mirror the existing spdk_ioat_submit_copy
and spdk_ioat_submit_fill. These new functions
*only* build the descriptors in the ring - they do
not write the doorbell. This enables batching
which can significantly improve performance by
reducing the number of MMIO writes.

This will enable batching of doorbell writes in
future commits. For now, just make the API public.
This is the first in a series of patches that
drastically improves performance for high queue
depth CB-DMA workloads. Some basic tests on
my Xeon E5-v3 platform shows about 4x improvement
for 512B transfers.

The ioctl NBD_SET_SOCK can return EBUSY on conditions not
only the kernel module hasn't loaded entirely yet, but
also the nbd device is setup by another process, which will
lead the poller's infinite polling.
This patch will wait only 1 second if device is busy.

Besides, the destructor of ftl_bdev will lock it again.

Added check before write submission to indicate if
LBA was update in meantime. In such case don't set band's
metadata and rwb entry cache bit. Previous implementation
invalidates such address during write completion and could
cause that inconsistent lba map was stored into disk.

We assumed io_channel allocation always succeeds, but
that's not true. Doing I/O to any vhost session that
failed to allocate an io_channel would most likely
cause a crash.
We'll now detect io_channel allocation failure and
print a proper error message. The SCSI target for
which the channel allocation failed simply won't be
visible to the vhost master. All I/O to that target
will be rejected.
We should probably report the error to the upper
layer and either prevent the device from starting
or fail the SCSI target hotplug request. But for now
let's just prevent the crash.

The helper function spdk_get_data_out_buffer_size() is a little
confusing because it does only returning macro constant
SPDK_ISCSI_MAX_RECV_DATA_SEGMENT_LENGTH.
The macro constant will be configurable and so the helper function
is not sustainable.
Replace the helper function simply by the macro constant.

When hotplug feature is enabled by NVMe driver, users may
call delete_nvme_controller() RPC to delete one controller,
however, the hotplug monitor will probe this controller
automaticlly and attach it back to NVMe driver. We added
a skip list, for those user deleted controllers so that
NVMe driver will not attach it again.
Fix issue #602.

User can create a probe context to probe and attach controllers
asynchronously, the controllers will be added to the context list
for the first step, then users can poll the context until the list
becomes empty.

Existing NVMe driver uses a global list g_nvme_init_ctrlrs
to track the controllers during initialization, and internal
function will start each controller in the list one by one
until the list is empty. We introduce a probe context
and move the global list into the context, with the context
we can enable asynchronous probe API in the next patch, also
this can enable parallel probe feature.

Previously, function spdk_nvme_probe_internal() will probe
NVMe controllers and then bring up probed controllers
into the ready state after that. Broke up original two parts
with probe and start stage, this will help us to introduce
a probe context in the next patch.

Althrough SPDK already provides a API to users which
can process runtime timeout NVMe commands, but it's
nice to have another API here, SPDK NVMe driver can
use it to break the endless wait. Also use the API
first in the initialization process, because we don't
want to add another initialization state with Intel
only supported log pages.

From TP8000 spec 7.4.7,
"In response to a C2HTermReq PDU, the host shall terminate the connection.
If the host does not terminate the connection in an implementation specific
period that does not exceed 30 seconds, the controller may terminate the
connection on its own".
It means that the timeout is designed for: when the target is
sending out C2hTermReq, if the host does not terminate the connection,
the target should terminate the connection.
PS: For detecting the malicous connection without sending response
(such as no response of R2T PDU) which should be another patch.

Currently, SPDK does not support vfio in no-IOMMU mode. However, it
seems quite easy to extend the vtophys code to add support for this.
vfio in no-IOMMU mode does not support DMA remapping. This implies that
physical DMA addresses are used instead of IOVAs.
This patch checks whether the vfio no-IOMMU mode is enabled using
function rte_vfio_noiommu_is_enabled() from the DPDK RTE vfio interface.
In this case, physical addresses are used for the DMA mappings. This is
the same code path for the DMA translations as when the uio is used as a
kernel driver.

This prevents issues where spdk_thread_poll may report
that it did not useful work (for the one poller it ran),
causing the system thread to go to sleep.
