per Intel policy to include file commit date using git cmd
below. The policy does not apply to non-Intel (C) notices.
git log --follow -C90% --format=%ad --date default <file> | tail -1
and then pull just the 4 digit year from the result.
Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc). Contribution date used
"--follow -C95%" to get the most accurate date.
Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line. Simply there for
consistency.

Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file. This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.
Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text. There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".
Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

This patch enables us to aggrete multiple ctrlrs in the same NVM
subsystem into a single bdev ctrlr to create multipath.
This patch has a critical limitation that ctrlrs which are aggregated
need to have no namespace. Hence any nvme bdev is not created.
However it will be removed in the next patch.
The design is as follows.
A nvme_bdev_ctrlr is created to aggregate multiple nvme_ctrlrs in
the same NVM subsystem. The name of the nvme_ctrlr is changed to be
the name of the nvme_bdev_ctrlr.
NVMe bdev module has both the failover feature and the multipath
feature now. To choose which of failover or multipath to use, add an new
parameter multipath to the RPC bdev_nvme_attach_controller.
When we attach a new trid to the existing nvme_bdev_ctrlr, we use the failover
feature if multipath is false, we use the multipath feature if multipath is
false.
nvme_bdev_ctrlr has a list for nvme_ctrlr and it is guarded by the
global mutex. Callers can query nvme_ctrlrs from a nvme_bdev_ctrlr via
trid as a key. nvme_bdev_ctrlr is not registered as io_device.

nvme_bdev_ns_to_bdev() was introduced because nvme_ns->bdev may be
removed to support multipath. However, it is unlikely now.
Hence it is easier to read to stop using nvme_bdev_ns_to_bdev() and
access nvme_ns->bdev directly.

In nvme_bdev_ctrlr_create, calloc will be called with a zero size
allocation request if the number of namespaces is zero. The behaviour
is implementation defined if the size of the space requested is zero -
calloc will either return a pointer that mustn't be dereferenced, or
NULL. If NULL is returned, the nvme_bdev_ctrlr_create will fail.
Only call calloc if there are a non-zero number of namespaces.
Otherwise, leave the namespaces pointer with a NULL value. All
references to namespaces[] are either known to be safe, or occur
in the context of looping through the namespaces which will be
skipped if the count is zero. The exception to this is in
vbdev_opal_create, where an assert has been added to match
equivalent code in bdev_ocssd_create_bdev.
Tested by running unit tests on a system that returns a null pointer
for a zero size allocation.

By the recent changes, the linked list nvme_ns->bdevs has only a
single bdev at the maximum. Hence replace it by the pointer
nvme_ns->bdev, and remove the linked list pointer nvme_bdev->tailq.

This patch removes the string from register component.
Removed are all instances in libs or hardcoded in apps.
Starting with this patch literal passed to register,
serves as name for the flag.
All instances of SPDK_LOG_* were replaced with just *
in lowercase.
No actual name change for flags occur in this patch.
Affected are SPDK_LOG_REGISTER_COMPONENT() and
SPDK_*LOG() macros.

For base bdevs, we maintain a global list g_opal_base, and
for part bdevs, there is also a global list g_opal_vbdev,
so it's not necessary to use the configuration again, just
unpack it to opal part bdev.

Commit 8c1d107 added a new erase API and renamed the original
API with "secure_" prefix according to the specification, as the
secure one will not cryptographically erase user data, due to
the limitation from the drive, only the secure API can be
supported, so changing the caller to use the original again.

The revert asynchronous API doesn't run as the *real* asynchronous
way, because the drive can only support synchronous module and only
1 session is supported. The reason why we added this API is that
RPC call has the default timeout value here, while the revert may
take over several minutes, the API itself doesn't short the revert
action, so just remove it and use the synchronous API instead.
The revert action will erase all the users data and bring the drive
back to the factory state, it should run in the synchronous mode,
so just remove the asynchronous API and we can increase the timeout
value when using RPC to call this API.

Init commit for opal vbdev. Opal vbdev is built based on
bdev part module. We split nvme namespace to spdk_bdev_part
with user specified offset and length. And then use this
configuration also to set up locking range for Opal.
Use linked list g_opal_vbdev to track all configuration for
each vbdev. And use g_opal_base to track the part_base for
all vbdevs.
This patch only shows the process for creating/deleting a vbdev
for an opal-enabled nvme ctrlr.
