In support of upcoming patches and to greatly simplify things,
the capabilites enum which held bit positions for each opcode
has been removed. Only the opcodes enum remains and thus only
opcodes are used throughout. For the capabiltiies bitmap a helper
function is added to convert from opcode to bit position. Right
now it is used in the IO path but in upcoming patches that goes away
and the conversion is only done at init time.

spdk_for_each_bdev() can be applied simply to rpc_bdev_get_bdevs()
because the callback function rpc_dump_bdev_info() is synchronous.
spdk_for_each_bdev() cannot be applied simply to rpc_bdev_get_iostat().
The factored-out callback function _bdev_get_device_stat() is
asynchronous. Add desc pointer to struct spdk_bdev_io_stat and open
before and close after executing spdk_bdev_get_device_stat().
Replace spdk_bdev_get_by_name() by spdk_bdev_open_ext() when a bdev name
is specified.
spdk_bdev_register() checks if the name of each bdev is set and each bdev
is opened while collecting its stats now. Hence it is not possible that
spdk_bdev_get_name() returns NULL. Simplify rpc_bdev_get_iostat_cb()
based on this fact.
Furthermore, we want to fail the RPC for all failures. The callback
function to spdk_bdev_get_device_stat() is executed after stack
unwinding if successful. Defer starting RPC response until it is ensured
that all spdk_bdev_get_device_stat() calls will succeed or there is no bdev.

RDMA READs can cause cmds to be submitted to the target
layer in a different order than they were received
from the host. Normally this is fine, but not for
fused commands.
So track fused commands as they reach
nvmf_rdma_request_process(). If we find a pair of
sequential commands that don't have valid FUSED settings
(i.e. NONE/SECOND, FIRST/NONE, FIRST/FIRST), we mark
the requests as "fused_failed" and will later fail them
just before they would be normally sent to the target
layer.
When we do find a pair of valid fused commands (FIRST
followed by SECOND), we will wait until both are
READY_TO_EXECUTE, and then submit them to the target
layer consecutively.

nvme_rdma_poll_events() gets the cm_channel pointer itself.
Before calling nvme_rdma_process_event(), we checks the

Hence we do not have to pass the cm_channel pointer to

This simplifies the code and makes the following patches a little easier.

Replace nvme_fabric_qpair_connect() by nvme_fabric_qpair_connect_async()
and nvme_fabric_qpair_connect_poll().
The following is a detail.
Define state of the nvme_rdma_qpair and each rqpair holds it.
Initialize rqpair->state by INVALID at nvme_rdma_ctrlr_create_qpair().
_nvme_rdma_ctrlr_connect_qpair() sets rqpair->state to
FABRIC_CONNECT_SEND instead of calling nvme_fabric_qpair_connect().
Then the new function nvme_rdma_ctrlr_connect_qpair_poll() calls
nvme_fabric_qpair_connect_async() at FABRIC_CONNECT_SEND and
nvme_fabric_qpair_connect_poll() until it returns 0 at FABRIC_CONNECT_POLL.
nvme_rdma_qpair_process_completions() or
nvme_rdma_poll_group_process_completions() calls
nvme_rdma_ctrlr_connect_qpair_poll() if qpair->state is CONECTING.
This patter follows the TCP transport.

Placement_id's are allocated first, then an empty one is found

R2Ts can cause cmds to be submitted to the target
layer in a different order than they were received
from the host. Normally this is fine, but not for
fused commands.
So track fused commands as they reach
nvmf_tcp_req_process(). If we find a pair of sequential
commands that don't have valid FUSED settings (i.e.
NONE/SECOND, FIRST/NONE, FIRST/FIRST), we mark the
requests as "fused_failed" and will later fail them
just before they would be normally sent to the target
layer.
When we do find a pair of valid fused commands (FIRST
followed by SECOND), we will wait until both are
READY_TO_EXECUTE, and then submit them to the target
layer consecutively.
This fixes issue #2428 for TCP transport.

but then fail GET_LOG_PAGE commands that specify
a non-zero offset, or report the wrong number
of discovery entries when reading more than the
discovery log page header but not the entire log
page.
So just revert to reading the entire discovery log
page, after we've read the header to know how big the
log page will be. This means that when we read the
log page initially (without the individual entries),
we need to save off the genctr, since it will get
overwritten when we read the log page again. We can
just store this in the discovery context, and compare
it to the genctr that we read with the whole log page.

Parameters submitted by bdev trace record may not be uint64_t
type (ex: bdev_io->type), which may bring outliers in trace parsing
results. The reason is that, in _spdk_trace_record() function,
va_list hold information about variable arguments, when passing
SPDK_TRACE_ARG_TYPE_INT arguments, it will call va_arg(vl, uint64_t)
to get value. However, if the submitted type is not uint64_t,
va_arg may return outlier.
To solve the problem, the parameter should be converted into
uint64_t before be delivered into trace record if it is not
uint64_t.

nvme_transport_ctrlr_connect_qpair() calls
nvme_transport_ctrlr_disconnect_qpair() if failed.
If async qpair disconnect is supported, even when connect qpair failed,
nvme_transport_ctrlr_connect_qpair() may complete asynchronously later.
The cases that qpair->async is set to true are I/O qpair for the NVMe
bdev module and admin qpair.
example/nvme/perf and example/nvme/reconnect use I/O qpair but both
set qpair->async to false.
For the NVMe bdev module, I/O qpair is connected when creating I/O
channel or resetting ctrlr. If spdk_nvme_ctrlr_connect_io_qpair()
returns 0 for a I/O qpair, the qpair is in a poll group and is polled
by spdk_nvme_poll_group_process_completions() and a disconnected
callback is called to the qpair. Hence we do not need to add additional
polling for I/O qpair in the NVMe bdev module.

spdk_nvme_poll_group has followed spdk_nvme_qpair about how to
process I/O qpair deletion inside of a completion context.
spdk_nvme_qpair_process_completions() accesses qpair after
returning from nvme_transport_qpair_process_completions().
So this is reasonable.
On the other hand, if spdk_nvme_poll_group_process_completions()
can execute spdk_nvme_ctrlr_free_io_qpair() inside of a completion
context, the target qpair is ensured to be deleted after returning
from spdk_nvme_ctrlr_free_io_qpair(). Then the target qpair is
not accessed anymore in spdk_nvme_poll_group_process_completions().
Remove two variables, in_completion_context and num_qpairs_to_delete,
of spdk_nvme_transport_poll_group and the related code.
This change is really necessary to support the following case.
In the NVMe bdev module, a nvme_qpair has a qpair and a poll_group
channel. disconnected_qpair_cb calls spdk_nvme_ctrlr_free_io_qpair()
for the qpair and spdk_put_io_channel() to the poll_group_channel.
spdk_nvme_ctrlr_free_io_qpair() is executed after unwinding stack
but spdk_put_io_channel() is executed now. The callback to
spdk_put_io_channel() calls spdk_nvme_poll_group_destroy(). However,
spdk_nvme_ctrlr_free_io_qpair() is not executed. Hence
spdk_nvme_poll_group_destroy() fails.

When generating a discovery log page, we will add entries
for the discovery subsystem for all listeners except the
one associated with the controller that generated the
log page command. We do this comparison using
spdk_nvme_transport_id_compare().
But this function compares the subnqn of the trid, and
the subnqn is not set in either of the trids that we
are comparing.
The listener's trid always has an empty subnqn, but
the source trid has an uninitialized subnqn when
we do the comparison. This means that sometimes the
subnqn may be empty (which always happens in debug
builds) but sometimes may contain garbage. This
means that sometimes an entry would be added to the
log, even for the trid of the discovery controller
that generated the command (meaning the discovery
controller would end up referring to itself which
is not allowed).
There is an even more subtle issue with this. If the
host reads just the log page header, the nvmf target
generates the entire log page, and just returns the
header contents. Let's say in this case, the source
trid has an empty subnqn, so we don't generate an entry
for it, and report numrec = X and genctr = Y. Then
the host reads the X log page entries. But now the
source trid is garbage, so a discovery log page entry
is returned, replacing one of the "real" log page
entries. And since genctr didn't change, the host
thinks the data is all valid, meaning there's a log
page entry for an NVM subsystem that ends up getting
dropped.

Then spdk_nvme_poll_group_process_completions() polls all qpairs in
the group->needs_poll list and hits the assert.

The root ports might have been configured by some other driver (e.g.
Linux kernel) prior to loading the SPDK one, so we need to clear it. We
need to before the scanning process, as it's depth-first, so when
scanning the initial root ports, the latter ones might still be using
stale configuration. This can lead to two bridges having the same
secondary/subordinate bus configuration, meaning that their config space
would map to the same memory area, which, of course, isn't correct.
This has manifested in issue #2413, where two root ports were configured
to use the same secondary bus. This caused an endpoint device to be
enumerated twice on two different root ports, with the first instance
being broken once the second port was configured by the SPDK driver.
