c9e90fd05e lib/iscsi: Use not double pointer but reference to pointer in iscsi_op_login()
Double pointer is clever but reference to pointer is easier to understand
because it may be familiar and reference to pointer has been used
for alloc_len and cid in the same function.

28dda0c016 lib/iscsi: Remove unnecessary declaration of R2T related functions
Group iscsi_send_r2t(), iscsi_send_r2t_recovery(), and
iscsi_remove_r2t_pdu_from_snack_list() to the following ordering:
 iscsi_send_r2t()
 iscsi_remove_r2t_pdu_from_snack_list()
 iscsi_send_r2t_recovery(),

move the group to just above add_transfer_task(), and then
remove declaration of iscsi_send_r2t() and iscsi_send_r2t_recovery().

This makes the structure of the source code more top-down.

a398d87467 lib/iscsi: Separate CHAP params setup and authentication process
This is a preparation to support per portal group CHAP authentication
for discovery session.

Previously require_chap, disable_chap, and mutual_chap had been set
and used in iscsi_negotiate_param(), and chap_group had been set
and used in iscsi_get_authinfo().

If a connection is in a discovery session, the connection can get
all CHAP params at its creation, spdk_iscsi_conn_construct().

If a connection is in a normal session, the connection can get all
CHAP params in iscsi_op_login_negotiate_chap_param().

Each connection is in either discovery session or normal session.

So the following change is possible and is done in this patch.

spdk_iscsi_conn_construct() sets all CHAP params of the connection
by global parameters. Then iscsi_op_login_negotiate_chap_param()
overwrites them by the corresponding target's parameters.
iscsi_negotiate_chap_param() and iscsi_get_authinfo() just refer
the CHAP params.

Besides, iscsi_get_authinfo() changed to call just
spdk_iscsi_chap_get_authinfo() inside, and so inline
spdk_iscsi_chap_get_authinfo() into iscsi_auth_params() and then
remove iscsi_get_authinfo().

portal_host and portal_name were added to struct spdk_iscsi_conn long
ago but had not been used yet. Portal group and its portals can be
removed dynamically even if there is any binded connection. Using
cached pg_tag will avoid potential errors.

1738ec7a9e iscsi: Simplify duplicated target pointers in iscsi_op_login_phase_none
conn->target is initialized to NULL in iscsi_op_login_phase_none
and then configured in iscsi_op_login_session_normal. Hence
subsequent references to target node can be done by conn->target and
related local variables can be removed.

fb641c4b54 iscsi: Assign connections to poll groups instead of lcores.
This patch binds poll groups to SPDK thread through IO channel and
assigns connections to poll groups instead of cores.

iSCSI subsystem registers iSCSI global object as an IO device, and create
poll groups as context of IO channels of the IO device.

Each portal get and hold portal group on which the corresponding acceptor is
running.

When a connection is constructed, iSCSI subsystem assigns a poll group
to the connection by getting it from the corresponding portal.

When a connection enters full-feature phase, iSCSI subsystem schedules
the connection to a poll group by round-robin.

Then, each connection can know its running SPDK thread directly and can
use SPDK message passing infrastructure instead of SPDK event framework.

By this change, iSCSI connections are binded to SPDK thread, and not
binded to processor core anymore.

Some other changes in this patch are
- core ID is removed from the output of get_iscsi_connections RPC. The
  upcoming patches will change the RPC to use spdk_for_each_channel and can
  access SPDK thread safely, and add SPDK thread ID instead.
- utilize UT multithread framework added by the last patch to test
  iSCSI poll groups by UT.

d1961b5e41 iscsi: Use not malloc'ed but fixed size string for target name and alias
Using malloc'ed string for string in iSCSI target has caused
scan-build error.

Maximum size of target name is already defined to be MAX_TARGET_NAME,
and hence use fixed size string whose size is MAX_TARGET_NAME + 1 for
spdk_iscsi_tgt_node::name.

Change psdk_iscsi_tgt_node::alias together.

This change will reduce the potential malloc failure.

beefcd1ae5 iscsi: Fix the bug to add wrong size in _iscsi_sgl_append_with_md
This bug was found by code inspection. When PDU read restarts
after data segment, iov_offset must be reduced by data length.
However iov_offset had been reduced by buffer length by mistake.

Lack of UT code was the main reason of this bug. Hence add UT
code to test the fix of the bug together in this patch.

94f67dd5ae dif: Change data_len of spdk_dif_set_md_interleave_iovs to be remaining length
This patch changes the meaning of the data_len parameter of
spdk_dif_set_md_interleave_iovs from `Expected data length of the payload`
to `Expected length of the newly read data in the extended LBA payload`.

This change will align the parameters of spdk_dif_set_md_intereleave_iovs
to of spdk_dif_generate_stream.

ba3adc4367 iscsi: return the correct error code in spdk_iscsi_read_pdu
We shoud not always return SPDK_ISCSI_CONNECTION_FATAL
in spdk_iscsi_read_pdu function.

Reason: In iscsi_conn_handle_incoming_pdus, the loop
directly return only rc==SPDK_ISCSI_CONNECTION_FATAL.
But it masks all the necessary information. So we would like
to keep some information of the return value for spdk_iscsi_read_pdu,
and we can use error log to track those information.

Then we can return SPDK_ISCSI_CONNECTION_FATAL as the error
return value for iscsi_conn_handle_incoming_pdus function.

37bdd0e87f scsi: Add data offset to DIF context separately from start block address
This patch uses the change by the last patch to initialize DIF
context in SCSI layer. Besides this patch changes the name of a
parameter from offset to data_offset to clarify the meaning.

3cfb1fe8dd iscsi: Schedule the connection only after completing login processing
Previously the connection was scheduled to another core just
after setting conn->full_feature to 1, but conn->state was still
ISCSI_CONN_STATE_INVALID. The connection started to run on another
core but could not run normally until conn->state became
ISCSI_CONN_STATE_RUNNING. conn->state was changed late to
ISCSI_CONN_STATE_RUNNING after sending the first login reponse.

This gap window had caused intermittent critical failures.

Based on this analysis, this patch changes to call
spdk_iscsi_conn_schedule() just after sending the final login response.
Whether any login response is final or not can be known by checking
conn->full_feature is not zero.

    Fixes #785

6d156d5b7d iscsi: Dump PDU contents to know what PDU was sent in illegal state
In some test cases, unknown iSCSI PDU other than the login request
has been sent to the iSCSI target when the iSCSI target is not in
runnable state, and it has caused failure in iSCSI target.

To know what PDU was received by the iSCSI target, this patch changes
the iSCSI taget to collect dump of the PDU.

SPDK has already the SPDK_LOGDUMP macro but the SPDK_ERRLOGDUMP macro
will be appropriate in this use case and added. Then the SPDK_ERRLOGDUMP
is used in iSCSI library.

We can decode PDU and output any format easy to read by human, but
creating good format is not easy task and error prone. So this patch
uses simple dump.

Dump outputs like:

2761079168 iscsi: Remove SPDK_SUCCESS and use 0 instead
SPDK_SUCCESS is a remnant and all other SPDK libraries have used
just 0. So this patch removes SPDK_SUCCESS from iSCSI library and
uses 0 instead.

a7d832f28c iscsi: Correct iovec increment when data digest is enabled
iscsi_sgl_append_with_md had not increment _iscsi_sgl::iov
by appended count of iovecs. Hence if data digest is enabled,
data segment will be overwritten by data digest.

This patch fixes the bug.

1204ff34e0 iscsi: Update _pdu only when completing read in spdk_iscsi_read_pdu
The only caller iscsi_conn_handle_incoming_pdus accesses the returned
pdu only when the return code is 1. So we can remove update of

spdk_iscsi_read_pdu has been used only in a place,
iscsi_conn_handle_incoming_pdus. iscsi_conn_handle_incoming_pdus
classifies return code of spdk_iscsi_read_pdu to
SPDK_ISCSI_CONNECTION_FATAL, 0, or 1.

Using 0 instead of SPDK_SUCCESS in spdk_iscsi_read_pdu matches
iscsi_conn_handle_incoming_pdus and is done in this patch.

f089b07af4 lib/iscsi: Rename _iov_ctx to _iscsi_sgl to match DIF library
Recently DIF library refined _iov_ctx to support multiple iovecs
for DIF insert and strip operations. iSCSI library is the first
user of DIF insert and strip, and so rename _iov_ctx to _iscsi_sgl
to match DIF library. Names of related helper functions and
a member variable are changed together.

This change will be helpful to extract common part into a specific
helper library if necessary in future.

2821762a60 dif: Support a single iovec for data buffer in spdk_dif_set_md_interleave_iovs
Currently iSCSI target have used a single contiguous data buffer
both for read and write I/O. Hence spdk_dif_set_md_interleave_iovs
accepts a single contiguous data buffer and its size as arguments.

On the other hand, NVMe-TCP target recently has changed to use
SGL data buffer instead.

DIF strip and insert will be supported in NVMe-TCP target next,
and updating spdk_dif_set_md_interleave_iovs to accept SGL data
buffer will be helpful.

This patch changes the interface of spdk_dif_set_md_interleave_iovs,
but allows only a single iovec. The next patch will allow multiple
iovecs.

8cc7b0bc4d lib/iscsi: Set destructed at the start of target node destruction
Add the flag destructed to struct spdk_iscsi_tgt_node and
iscsi_op_login_check_target() refers it and returns
ISCSI_LOGIN_TARGET_REMOVED if it is true.

When destructing iSCSI target node, it will be nice if iSCSI library
can stop further connections are created but, connections are not
associated with any target node until processing login. Hence stop
creating sessions instead.

Additionally, when destructing iSCSI target node, if the flag destructed
is already set, return immediately, and the flag destructed does not
affect discovery session.

7b266460d0 iscsi: handle aborts for queued_datain_task that is completed
It's possible that we get a request to abort I/O just after
a queued_datain_task has completed, but before we've had a
chance to remove it from the queued_datain_tasks TAILQ.
_iscsi_conn_abort_queued_datain_task wasn't accounting for
that which would result in an infinite loop.

Fixes issue #725.

5a1dc44423 iscsi: Fix LUN ID in Ready to Transfer PDUs
LUN ID is not converted from integer to structure and integer is
set to R2T PDUs.

Popular iSCSI initiators don't check this value and work correctly.

This patch uses the public helper function of the SCSI library
to fix the issue.

Additionally, private helper function to convert structure to
integer for LUN ID is replaced to the public helper function
of the SCSI library.

893e02a54f iscsi: Remove the prefix spdk_ from names of private functions
iSCSI library had used the prefix spdk_ for most functions regardless
of private or public. Using the prefix spdk_ only for public functions
will be helpful to distinguish private and public functions, and
will be helpful to investigate issues or do further improvement.

Besides in iscsi.c static variable spdk_arc4random_initialized had
the prefix spdk_, and change it to g_arc4random_initialized according
to the SPDK's good practice.

iSCSI library still have some issues but is more stable than before
and now will be the good time to adjust the naming rule to other
libraries.

This patch doesn't change any behavior.

1bec7d572c iscsi: Fix the bug when querying DIF context for Data In PDU
spdk_iscsi_get_dif_ctx() didn't work correctly for Data In PDU
because we had to get iSCSI task from PDU directly for Data In PDU,
and lun_id was not copied in spdk_iscsi_task_get().

This patch fixes these bugs and the DIF strip feature was verified
after applying this patch.

136c3fb461 iscsi: Generate and verify DIF to metadata space in read or write I/O
Generate and insert DIF for write I/O by stream fashion in
spdk_iscsi_read_pdu().

Verify DIF for read I/O in spdk_iscsi_conn_write_pdu().

Verification can be done in spdk_iscsi_build_iovs(), but how many
writes to socket is not known beforehand, and same range may be
verified multiple times. So verify DIF before starting to write to
socket.

daf2206ec4 iscsi: Skip metadata space in read or write data from network socket
This patch leaves metadata space in data buffer for read or write I/O
to generate and verify DIF.

For write I/O,
- in spdk_iscsi_read_pdu(), leave metadata space for every block
  by calling spdk_dif_set_md_interleave_iovs() and then
  spdk_iscsi_conn_readv_data(), and remember this in PDU.
- in spdk_iscsi_op_scsi() and spdk_iscsi_op_data(), set not data
  length but buffer length to length of an iovec. The reason is
  that the actual data length sent to SCSI layer will be larger
  than the data length set in PDU. SCSI task holds data length
  as an independent member and this change doesn't loose any
  information.

For read I/O, in spdk_iscsi_build_iovs(), leave metadata space for
every block by calling spdk_dif_set_md_interleave_iovs().

The reason why DIF context is saved in PDU is that DIF verification
will be done in spdk_iscsi_conn_write_pdu() and DIF context will be
used there first.

c24f506993 iscsi: Increase the size of write buffers to store DIF
Increase the size of write (data out) buffers and immediate
data buffers in the iSCSI library to store DIF. Increase is the
amount necessary to store 16 byte metadata block to the block
formatted iwth 512 + 16. 512 + 16 is the current maximum ratio
of metadata per block.

Use the macro SPDK_BDEV_BUF_SIZE_WITH_MD defined in
include/spdk/bdev.h for this purpose.

Besides, change the name of a helper function from
spdk_get_immediate_data_buffer_size() to
spdk_get_max_immediate_data_size() to differentiate the maximum
data size and the buffer size, and then to use the macro.

b8a51ff5b8 iscsi: Add an API to return DIF context of SCSI command
Ths patch adds an API spdk_iscsi_get_dif_ctx().

When opcode of BHS is SCSI command, spdk_iscsi_get_dif_ctx() extracts
CDB and LUN ID from BHS. When opcode of BHS is Data Out or Data In,
spdk_iscsi_get_dif_ctx() extracts TTT and get R2T task corresponding
to TTT, and gets CDB and LUN ID from the task.

Then spdk_iscsi_get_dif_ctx() gets LUN by using LUN ID and calls
spdk_scsi_lun_get_dif_ctx().

Additionally spdk_iscsi_get_dif_ctx() checks the state of the connection
and the type of the session is valid before starting decode operation
because contents of PDU is not reliable when the state of the connection
is invalid. The necessity of the check was found by the Calsoft test tool.
Checks and test cases are not matched yet but copying existing checks
from spdk_iscsi_conn_execute, spdk_iscsi_op_scsi, and spdk_iscsi_op_data
will be enough. The check added for Data Out is not necessary for Data In
but it will not be harmful for Data In.

e17851314e iscsi: Hold size of buffer to store PDU data segment in struct spdk_iscsi_pdu
Upcoming patches will store larger data in the data buffer than
iSCSI Initiator transfers, and size of metadata is not known until

fa7dd957ee iscsi: Reduce required free iovecs from 5 to 1 to flush PDUs
Upcoming patches will support DIF insert and strip feature in
iSCSI target and the feature will be implemented by utilizing
iovecs.

Even when we support the DIF feature, we want to keep current
batched PDU flush, and current requirement that there must be
enough free iovecs to map all segments of a PDU is too strict.

This patch alleviates the requirement by passing remaining
number of iovecs to spdk_iscsi_build_iovs().

879386d759 iscsi: Unify build and fast-forward iovecs to flush PDUs
Building iovecs had been done in spdk_iscsi_build_iovs() and
fast-forwarding iovecs for the partially written first PDU had
been done in-line separately.

This patch unifies these two operations into spdk_iscsi_build_iovs().

Fast-forwarding iovecs is necessary only for the first PDU, but the
operation is applied to all PDUs after this patch.

Extra overhead will be negligible because usually at most two
iovecs are consumed, one is for the base header segment and another
is for the data segment.

42596fb6b1 iscsi: Use iov instead of iovec for names of variabls and functions
iovec_cnt and iovec_array are very descriptive and good but iovcnt
and iovs are often seen in SPDK and will be enough.

Subsequent patches will add some changes on iovec operations and
simple and familiar names will be helpful to work and review them.

This patch doesn't change any behavior.

confusing because it does only returning macro constant

is not sustainable.

8827ee1ed2 iscsi: remove ExpStatSN check
Technically this check is correct, but the Linux kernel
target doesn't have it, and older versions of libiscsi
have a bug which result in stale ExpStatSN getting sent
resulting in terminated connections with the SPDK iSCSI
target at high queue depths.

Fixes #600.

eb6bac0aa9 iscsi: Fix the issue caused by cleanup LUNs when exiting connection
When exiting connection, all active tasks are aborted by
spdk_iscsi_tgt_node_cleanup() in lib/iscsi/tgt_node.c.

In spdk_iscsi_tgt_node_cleanup(), spdk_iscsi_op_abort_task_set()
is called by using a temporary task without associating PDU.

This call of spdk_iscsi_op_abort_task_set() have resulted in
assertion in spdk_iscsi_conn_abort_queued_datain_tasks() because
task->pdu is NULL.

The fix is to remove the assertion in
spdk_iscsi_conn_abort_queued_datain_tasks() and add check if
pdu == NULL as true condition.

This issue was detected by ip_migration test. ip_migration test
runs only in nightly tests now but it consumes only 26 seconds,
and the next patch moves ip_migration test to per-patch tests.

For split SCSI read command, if there is any failure in the sequence of
it, the first error must be propagated to all subtasks of it.

For split SCSI write command, if there is any failure in the sequence
of it, the first error must be propagated to the primary subtask.

Before this patch,
for read task:
- any failure is propagated to already completed subtasks, but
  is not propagated to any subtasks not completed yet, and
- if any failure occurs in non-primary subtasks, it is not propagated
  to the primary subtask.

for write task:
- if the primary subtask completes after any failure of non-primary
  subtasks, the failure will be overwritten by the success of the
  primary task.

This patch fixes these issues.

943561fae1 iscsi: Simplify the check of return value of spdk_iscsi_reject()
The possible cause of failure in spdk_iscsi_reject() is only -ENOMEM,
and SPDK_SUCCESS is 0 and SPDK_ISCSI_CONNECTION_FATAL is -1.

Hence just calling spdk_iscsi_reject() as return value will be
enough.

Besides, one redundant return path was found and fixed in this
patch too.

308b62c6ba iscsi: Fix uncorrect LUN reset called when exiting connection.
This direct call of LUN reset has caused unexpected behavior
during heavy stress test.

Apply correct LUN reset handling for this case too.
Even if there are multiple LUNs, LUN resets are serialized in
the SCSI layer by previous patches and parallel calls of LUN resets
will not cause any issue.

5124f87f4e iscsi: Abort queued datain task before submitting abort to SCSI layer
By up to the previous patch in the patch series, unexpected behavior
due to write tasks in task management commands have been fixed.

But unexpected behavior due to read tasks in task management commands
have been still observed.

Remaining patches in the patch series will fix the unexpected behavior
due to read tasks in task management commands.

This patch is for ABORT TASK.

ABORT TASK is not supported in SCSI layer yet. But the initiator
doesn't care about the failure is due to not-supported or failure.
It must be avoided that the task management command returns SCSI Good
but some tasks are not aborted and return SCSI Good later. On the other
hand, it is acceptable that the task management command returns
failure but some tasks are partially aborted.

Hence this patch adds operation without checking the support status
in SCSI layer.

728972cdf3 scsi: Set TMF code prior to calling spdk_scsi_dev_queue_mgmt_task
By subsequent patches for iSCSI, spdk_scsi_dev_queue_mgmt_task()
will not be called directly from the function that knows TMF code,
and currently setting TMF code to SCSI task is done in
spdk_scsi_dev_queue_mgmt_task().

Hence after subsequent patches for iSCSI, to hand off TMF code to
SCSI task, any dynamic context will be required.

To avoid the dynamic context, extract setting TMF code from
spdk_scsi_dev_queue_mgmt_task() and put appropriate place for
each call of spdk_scsi_dev_queue_mgmt_task().

Additionally, in spdk_abort_transfer_task_in_task_mgmt_resp(),
ref_task_tag is got from PDU but getting it from SCSI task is
much easier. Hence get ref_task_tag from SCSI task in the callback.

6dd09113e5 scsi: Serialize LUN reset execution
Task management functions don't require performance. Serializing
execution of task managment functions will sipmlify and stabilize
the logic and will be helpful for upcoming patches to support
other task management functions.

This patch introduces two queues, pending and submitted queue,
and serializes LUN reset exection and makes LUN hot removal
wait for LUN reset.

Besides, checking if LUN is NULL is moved to the upper layer as
same as IO task submission.

a115177b98 iscsi: Clear only R2T tasks prior to task management request
As described in RFC3720, task management request will act on all
the commands from the same session having a CmdSN lower than the
task management CmdSN.

Current implementation clears all R2T tasks without checking CmdSN.

This patch changes the implementation to clear only R2T tasks
whose CmdSN is smaller than the task management request.
Additionally this patch adds to UT code to verify the change works
as expected.

f550dbe893 iscsi: Clear R2T tasks after completing SCSI layer in task management request
Current immediate calls of clear_all_transfer_task() and del_transfer_task()
will cause unexpected behavior during heavy workload which cause repeated
reset operation.

Observed behavior is similar to Github issue #457.

38dfce0428 scsi: add iSCSI initiator port TransportID
SCSI persistent reservation feature need to get the TransportID
for the I_T nexus, so when creating initiator port we also
set the TransportID according to the specification, while here,
we use the format code 0x1 for the TransportID.

In RFC3720, the following terminologies based on RFC1994 are used:

- CHAP_C=<Challenge>
- CHAP_N=<Name>
- CHAP_R=<Response>
- CHAP_I=<Identifier>

The next patche will support base64 constants. Sticking to the terminologies
will be helpful to review and maintain it.

855633a597 iscsi: Get LUN ID from task instead of Data-Out PDU to fix SCSI compliance test failure
A major SCSI compliance test expects that the LUN field is reserved
(not to be checked) in the SCSI Data-Out PDU.

In that test, the LUN field in the first SCSI Command PDU is 0 and
the LUN field in the SCSI Data-Out PDU sent in response to the R2T
is 0xFFFFFFFFFFFFFFFF. The TTT field in the SCSI Data-Out PDU is 0xF that
is copied from the R2T. 0xF is valid as TTT.

Hence spdk_iscsi_op_data() returns Check Condition because LUN is
not found.

This patch changes to get LUN ID from not the LUN field in the SCSI
Data-Out PDU but the saved task.

The following is excerpts from iSCSI specification (RFC7143). The
behavior of the SCSI compliance test looks against the specification
but changing SPDK will be safer.

11.7.4.  Target Transfer Tag and LUN

   On outgoing data, the Target Transfer Tag is provided to the target
   if the transfer is honoring an R2T.  In this case, the Target
   Transfer Tag field is a replica of the Target Transfer Tag provided
   with the R2T.

   On incoming data, the Target Transfer Tag and LUN MUST be provided by
   the target if the A bit is set to 1; otherwise, they are reserved.
   The Target Transfer Tag and LUN are copied by the initiator into the
   SNACK of type DataACK that it issues as a result of receiving a SCSI
   Data-In PDU with the A bit set to 1.

   The Target Transfer Tag values are not specified by this protocol,
   except that the value 0xffffffff is reserved and means that the
   Target Transfer Tag is not supplied.  If the Target Transfer Tag is
   provided, then the LUN field MUST hold a valid value and be
   consistent with whatever was specified with the command; otherwise,
   the LUN field is reserved.

b6ab3116fc iscsi: Load CHAP secrets from file once at boot and use them in memory
For secure iSCSI targets, dynamic reconfiguration of CHAP secrets is
must to have.

Currently CHAP secrets are loaded for every CHAP authentication
operation. The current implementation will not work correctly
when CHAP secrets in the file are changed dynamically.

If SPDK loads CHAP secrets from the file only at boot and they can be
configured by RPCs, user can change CHAP secrets safely during run time.

Even if there are any users who expect dynamic reconfiguration of
CHAP secrets based on the current implementation, if we provide
this better alternative based on RPCs, they will be able to continue to
satisfy their requirement.

This patch change the current implementation so that SPDK loads
CHAP secrets from the file once at boot and uses them in memory
hereafter.

Besides, use fixed size buffers to hold CHAP secrets.
Previously dynamically allocated buffers by strdup() had been used,
but it required many nomem checks and should be avoided.
Other iSCSI targets/initiators have used fixed size buffers and SPDK
follows others.

Set the size of buffers for both user names and secrets to 256 (the
last byte is for NULL termination). 256 is sufficiently large
compared with others.

CHANGELOG will be updated in the separate patch because new RPCs will
be added instead.

d393983d74 lib/iscsi: export FirstBurstLength as user configurable parameter
According to https://tools.ietf.org/html/rfc3720, the
default value for the FirstBurstLength is 65536 bytes
while SPDK iSCSI target picks the smaller 8192 as the
default setting. This value is the communication for
the iSCSI initiator to send the unsolicited data and
instead of having a fixed setting here, expose it as a
user configurable parameter to fit the real use case,
especially for the data out iSCSI write.

Example of usage as following in the iSCSI.conf:

FirstBurstLength 8192

1b987076e4 iscsi: Use chap to variable names for CHAP in discovery session
Current variable names for CHAP in discovery session may not be
clear and long compared with CHAP in login to target nodes.

This patch changes variable names for CHAP in discovery session
from no_discovery_auth, req_discovery_auth, req_discovery_auth_mutual,
and discovery_auth_group to disable_chap, require_chap, mutual_chap,
and chap_group, respectively.

By this patch, the term "discovery" is removed but this will not
cause any confusion because the code of the use case of them is
clear and an new RPC set_iscsi_discovery_auth will be added in the
subsequent patches.

c8fd001075 iscsi: fix the primary iscsi task free in queued_datain_tasks
Reason: If the task is queued in scsi layer to handle, we
should free the task after calling the spdk_iscsi_task_cpl.
Otherwise, if only this task is executed (without subread tasks),
the task will be freed by the loop early, which will cause
the segment fault in spdk_iscsi_task_cpl function.

cefab5d647 iscsi: Support hot removal of LUN based on LUN open/close
When hot removal of a LUN is started, callback is called for each
iSCSI connection which accesses the LUN. Callback checks all transfer
tasks complete and then close the LUN.

If the connection clears all transfer tasks before getting all responses
to them from the initiator, the initiator continues to retry data write.
Hence the connection have to wait until all transfer tasks complete.

c5b00933ce iscsi: Fix the issue that queued iSCSI tasks are not migrated when clearing tasks
When multiple LUNs are attached and spdk_clear_all_transfer_tasks() is called with
a LUN, all tasks whose target is the LUN are removed and there will be room to start
queued tasks for other LUNs. However currently no migration is done.

Add UT code to verify the fix too.

This bug was caused by the following two changes:

iscsi: Remove duplication of the variable for write completion to bdev

iscsi: restore data_transferred accumulation for read

For write, bytes_completed is always equal to data_transferred. However,
for read, bytes_completed is based on expected data transfer length and
data_transferred is actual read size. Hence bytes_completed cannot be
used to calculate residual counts.

One of the reason why this bug cannot be found was that there was not any
test case when task->scsi.data_transferred is 0 and task->scsi.length is
not 0. Hence UT code is also added.

Same bug will occur for read sense data. Hence UT code for read sense data
is added in the next patch.

96e930fc13 iscsi: Fix the potential core dump issue for hotplug test case.
when test hotplug case with large write command which need r2t, after the
device hot removed, the outstanding r2t commands are waitting for data out PDU,
and the primary commands probably already return with error, then the lun
could be deleted, later the related data out PDU will received and try to send
other r2t, this patch make sure the r2t command sent smoothly even the device
hot removed.

60f1d52605 iscsi: restore data_transferred accumulation for read
In the patch 393582, by analyzing the code,
accumulation of write completion to bdev was duplicated by
data_transferred and bytes_completed.

Hence accumulation of data_transferred for write was intended
to be removed.

However by mistake accumulation of data_transferred for read
was removed.

In this patch restore accumulation of data_transferred for
read and remove accumlation of data_transferred for write.

Test code to catch this degradation will be added by another
patch soon.

8bb603da82 iscsi: Remove duplication of the variable for write completion to bdev
This patch will follow the change 393212 by Daniel.

For read command:
- primary's scsi.bytes_completed is for read completion from bdev.
- primary's scsi.data_transferred si for send completion to initiator.

Two variables have its own meaning, respectively.

However for write command:
- both primary's scsi.bytes_completed and scsi.data_transferred are
  for write completion to bdev.

Hence for write we should use only either one and it looks that
bytes_completed will be appropriate.

41bfe2049d iscsi: Sequence error of Data-out PDU when ERL=0
SPDK drops the connection when it observes any sequence error of
Data-out PDU when ERL = 0. This is too severe and change dropping
the connection to Reject response.

This issue was detected by libiscsi test suite.

59970a89be astyle: enforce braces around single-line statements
Require braces around all conditional statements, e.g.:

    if (cond)
        statement();

becomes:

    if (cond) {
        statement();
    }

This is the style used through most of the SPDK code, but several
exceptions crept in over time.  Add the astyle option to make sure we
are consistent.

02814cd34b iscsi: Change protcol error from drop conn to reject
In iSCSI specification RFC7143, there is the following description
in the section 7.13:

  All violations of iSCSI PDU exchange sequences specified in this
  document are also protocol errors.

Any protocol error should be handled as iSCSI Reject response whose
the reject reason is Protocol Error.

However current implementation handles a minor PDU error as Drop
connection. This is too severe and change the code from Drop
connection to Reject response.

This issue was detected by libiscsi test suite.

Disambiguate the log components from the trace functionality
(include/spdk/trace.h).

The internal spdk_trace_flag structure and related functions will be
renamed in a later commit - this is just a find and replace on

The following functions returns 1 and 0 for succcess and error,
respectively:

Using bool for this purpose will avoid our misunderstanding.

d97476efdf iscsi: shutdown iscsi library as part of subsystem fini path
With the new asynchronous subsystem finish framework, we can
drive shutdown of existing connections as part of the subsystem
finish path instead of a separate spdk_iscsi_shutdown function
called as the shutdown function in response to SIGINT.

This is a step towards enabling a single target app that
supports multiple protocols (i.e. iSCSI + vhost + NVMe-oF).

ed2f2d9d0e iscsi: remove configuration for unsupported parameters
Our iSCSI target does not support disabling InitialR2T,
DataPDUInOrder and DataSequenceInOrder, and will fail
if someone tries to disable them in the config file.

So instead, just do not support these parameters at all.
This simplifies the code and reduces confusion.

1e33a802b4 iscsi: fix layout of logout request reason field
The Logout Request reason field is the low 7 bits of byte byte 1; the
last bit of byte 1 is specified to be always 1, and we shouldn't
consider it to be part of the reason field.

The existing debug print code was parsing the reason field correctly (by
masking it against 0x7f), but it's simpler to just make the reason field
into a bitfield of the proper size; this fixes the real bug in the
reqh->reason != 0 check, which did not use the 0x7f mask.

Fixes GitHub issue #198.

40b74cd343 iscsi: optimization of large read process.
This patch move the spdk_iscsi_conn_handle_queued_tasks() to the main
loop of connection poller, make the logic more clear, it will also fix
one issue of hotplug:
for large read command, it has the potential risk:
one task will be split into N subtaks, when primary task return error and
try to send data in pdu, it will call spdk_iscsi_conn_handle_queued_tasks(),
the primary task is pending now, and all the subtasks will return error from
lun layer synchronous, this make the primary task return from the function
spdk_iscsi_transfer_in() after all the other subtaks, but when the N - 1 subtask
return from function spdk_iscsi_transfer_in(), it meet the condition:
primary->bytes_completed == primary->scsi.transfer_len
then it will send response pdu, after this, the primary task return from
spdk_iscsi_transfer_in(), it also meet the condition, so it will also try
to send response pdu, this will make the application run into error.

These variable names are changed to retain conformity with astyle 3.0.1. Astyle 3.0.1 recognizes 'final' as a keyword and incorrectly formats the iscsi_spec.h file breaking the build. Names were changed to avoid this case and maintain consistency within the struct.

Factor out the common PDU header and data digest calculations into
helper functions in iscsi.c.  This eliminates the need for the crc32c.c
iSCSI-specific padding functions spdk_fixup_crc32c() and spdk_crc32c(),

cce22074a7 iscsi: fix ISID interpretation in login requests
The ISID field is a 6-byte field in network (big-endian) byte order.

The previous code was casting the uint8_t isid[6] value into uint64_t,
which was actually casting the address of the first byte of isid (not
the contents of the array), and it was also not correctly converting
byte order.

144065f30c iscsi: fix big READ task release process for ERL1
When we enabled the ERL1 configuation, for the DATAIN task release
process, we will queue the task to the SNACK list firstly, and then
remove the list when got ACK from initiator, but for this part of
logic, the reference count of primary task was not released correctly.

d27b24c94b log: split internal TRACELOG macro into new header
The SPDK_TRACELOG macro depends on a CONFIG setting (DEBUG), so it
should not be part of the public API.

Create a new include/spdk_internal directory for headers that should
only be used within SPDK, not exported for public use.

f30f0c76f1 scsi: refactor usage of iov from spdk_scsi_task
This patch is preparation for fixing alloc_len overrun in SENSE 6/10 and
READCAP 6/10. To simplify code forbid usage of iov outside of
scsi/task.c.

This also drop SPDK_SCSI_TASK_ALLOC_BUFFER flag that obfuscate code. As
a replacement assume that if field alloc_len is non zero it mean that
iov.buffer is internally allocated. Functions
spdk_scsi_task_free_data(), spdk_scsi_task_set_data() and
spdk_scsi_task_alloc_data() manage this field.

a2d0fa5351 spdk: add support for readv and writev in scsi layer
This patch adds support for spdk_bdev_readv in scsi layer.
It also fixes write so that it uses multiple iov's instead of one.
Currently we should use only task->iov (for single vector operation)
or task->iovs (for multiple vector operations).
