The log priority was very syslog specific. Instead, create
a generic set of log levels as an enum.
The log level/priority isn't actually used anywhere today.

Much like bdev modules inside the bdev directory,
add a subsystems directory inside of event. The subsystem
specific code for the bdev library is placed in to
a separate library in that directory, breaking the
strict dependency of the bdev library on the event subsystem
code.

Now that virtual mode namespaces can be chosen arbitrarily, there can be
holes in the ns_list. Make sure all of the virtual mode code paths are
prepared to handle these inactive but valid NSIDs.

We can do all of the completion work first, and then
make the decision at the very end on whether to
defer the callback or not.
This also removes the bdev_io defer_callback member -
we no longer need it since we now only defer the
callback itself, instead of deferring the full
execution of the spdk_bdev_io_complete() routine.

In this patch, spdk_app_init will be merged into
spdk_app_start.
Due to this change, we need to change event_perf
to use spdk_app_start, since we cannot use spdk_app_init
anymore. So the related changes for event_perf is to
make it work with reactor framework.

This patch shows how to use the spdk_mock library which now only
consists of --wrap capability for a few functions.
Next will be a patch that provides a generic mock capability to
use either the --wrap feature or a combination of globals and
stubs to make adding more UT easier wrt mocking. After that is
in place nvme_ut.c will be updated to use the new library for
all mocking.

This patch doesn't use it yet but sets up the ability to so that
upcoming unit test pathces can. The next patch will include use
of wrap in one nvme UT and then following that the wrap and
today's use of globally controlled stubs will be morphed into a
simple mock type capability for spdk unit tests.

It is not actually useful to be immediately returned
a handle to the bdev_io. There isn't anything valid
that the user can do with it at that point. Instead,
return an integer error code.

Theres no need to have vhost SCSI (and incoming vhost block) structures
available in public API. Generic spdk_vhost_dev might be used to
communicate with backedns. Backends are responsible to check if type of
passed vdev valid.

Dynamically allocate bdev names to remove the arbitrary 16-character
name length limit.
All of the existing product_names are constant strings, so those can
just use string literals instead of a copy per bdev.

This patch adds property of transport protocol id to spdk_scsi_dev.
This allows to change it depending on which appliation is using
the device. Previously only iSCSI was used for all.
Setting protocol id is done at the time when device is added to
vhost controller (vhost) or target node (iSCSI).
Please note that for SPDK vhost SAS protocol id is used,
as that is what kernel vhost reports as well.

Moved controller-related variables from spdk_vhost_scsi_controller to
spdk_vhost_dev. Generalized vhost to operate on non scsi-specific
controller struct.
Now spdk_vhost_scsi_dev contains direct spdk_vhost_dev field instead
of a pointer. This means there is no more allocation/deallocation
on poller start/stop.

This ensures that all spdk_bdev_io structures now have
an attached channel, simplifying some future work around
things like counting the number of outstanding IOs for
a given channel (which otherwise would have had to
account specially for resets).
Reset semantics are still that they affect the entire bdev
and not just the channel it was submitted on.

The user should not see the bdev_io status directly; the NVMe and SCSI
error code wrappers provide the ability to translate to the desired
format regardless of what kind of error is stored inside the bdev_io.
Replace the spdk_bdev_io_completion_cb status parameter with a bool
simply indiciating whether the I/O completed successfully.

An ECN to the NVMe 1.3 specification has clarified that the NQN may
contain 223 bytes before the null terminator. Make all of our NQN
length checks consistently enforce this behavior.

Scan the source for POSIX includes outside of the
allowed locations in check_format.sh. This only
tests for POSIX headers - not Linux Standards Base.
Also, fix one bug that was caught by this addition.

The SCSI layer was not using the task ID for anything; the iSCSI layer
was using it to store the task tag, so move it there and rename it to
"tag" to make its purpose clear.

In the pattern set by spdk_bdev_io_complete_nvme_status(), allow
blockdev modules to complete a bdev_io with a SCSI status code.
Also move it to the internal bdev header file, since only bdev modules
should be setting bdev_io status codes.
