For the purpose to support shared IO CQ feature, we will construct the queue pair data structure at the beginning, and setup SQ/CQ separately in CREATE IO SQ/CQ routine. Previously we will disconnect queue pair when got a DELETE IO CQ command, now we disconnect queue pair when got a DELETE IO SQ command, and in the disconnect completion callback, we will release the IO SQ resources, there is a case that the VM will just RESET/SHUTDOWN controller when IO queue pairs are connected, for this case, we will also try to release CQ resources in the disconnect completion callback. `free_qp` function now is only called when destroying a controller.
