Performing a subsystem update on each poll group after the addition or removal of a namespace allows us to avoid the case where we pause a subsystem, perform a removal, then an addition, resume the subsystem and don't update the channel for that subsystem. This, however requires that spdk_nvmf_subsystem_remove_ns be asynchronous.
