First submission. Implemented part of the Opal library
and "scan" function. Can be invoked by nvme_manage.

SCSI persistent reservation feature need to get the TransportID
for the I_T nexus, so when creating initiator port we also
set the TransportID according to the specification, while here,
we use the format code 0x1 for the TransportID.

related parameter definition based on spc3r23 specification. After
that, we will add persistent reservation support in SCSI layer.

Automatically detect more whitespace errors.
All existing cases are fixed; only whitespace change (verify with
diff -w) except for one comment style fixup in include/spdk/nvme.h.

The SCSI spec defines two separate fields within the first byte of
the Inquiry data. Define them as bitfields and add enum values for the
peripheral qualifier field.

This patch assigns correct value to page control.
Now that page control value is correctly taken from CDB,
error via sense data is reported when processing "saved values".
"Changeable values" are not supported, so all parameters
are reported as not changeable when requested.
