This patch is a preparation for dynamic reconfiguration of PG-IG maps.
Current PG-IG map is implemented by a fixed-size array. Linked list
will make much easier to support dynamic reconfiguration of PG-IG maps.
Current:
target - (1..n) ---> PG_map (1..1) -> PG
|
--> IG_map (1..1) -> IG
This proposal:
target - (1..n) -> PG_map --- (1..1) -> PG
|
-- (1..m) -> IG_map - (1..1) -> IG

Current fixed sized array for iSCSI target is not thread safe.
When newly created targets are inserted into the array, multiple
targets may be inserted into the same position. Linked list is
better than fixed size array to support dynamic reconfiguration.

To add initiators to an existing target, manage name and netmask
of initiator group by linked list. It is not easy for array to
detect duplication and change configuration.

Group the code fragments of add/delete name and mask of initiators
and create spdk_iscsi_init_grp_add/delete_initiators/netmasks()
functions. Memory alloc/free is done in these functions.

This will enable asynchronous request handling in a future patch, and it
also removes the need for the RPC handlers to know about request id and
the JSON-RPC rules about notification-only requests.

Fixed double free in spdk_rpc_add_portal_group()
spdk_iscsi_portal_create() now takes string arguments as const char* and makes internal copies of them.
This patch also fixes potential memory leak when id == NULL

The SPDK_TRACELOG macro depends on a CONFIG setting (DEBUG), so it
should not be part of the public API.
Create a new include/spdk_internal directory for headers that should
only be used within SPDK, not exported for public use.

The lcore_id field in the get_iscsi_connections RPC was removed in
commit 5d8c94536a7d1d4c1f0ee3349188bf0e7e8c9e74; add a field to
spdk_iscsi_conn to track the lcore so this can be re-added.

This leaves more flexibility for future changes to the poller
representation without requiring API changes (after this one).
It also prevents the user from accidentally using poller fields in a
non-thread-safe way, since they can't be accessed directly anymore.
