per Intel policy to include file commit date using git cmd
below. The policy does not apply to non-Intel (C) notices.
git log --follow -C90% --format=%ad --date default <file> | tail -1
and then pull just the 4 digit year from the result.
Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc). Contribution date used
"--follow -C95%" to get the most accurate date.
Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line. Simply there for
consistency.

Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file. This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.
Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text. There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".
Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

An NQN can only be a maximum of 223 bytes, but the
field containing the NQN in all of the NVMe data
structures is 256. Specify that #define so it can
be used in data structures that need it.
Upcoming muser transport will also use this to
facilitate nqn handling.

It is the first patch to follow the NVMe over fabrics
spec and implmenent the NVMe/TCP transport. It can be
divided into work in the host and target sides:
Host side: Add the TCP/IP transport in nvme lib (lib/nvme).
Target side: Add the TCP/IP transport in nvmf lib (lib/nvmf).

NVMF spec lists that the property get commnad and set command
attributes bits 7:3 is reserved, bits 2:0 is used for the size
of property to return.
So update the codes for nvmf specification compliance.

Added tighter regulations to the NVMe-oF nqn checking to conform with
the nvme 1.3 spec. including, adding checks for valid nqn's in the case
of a generic uuid based nqn and checking for reverse domain name and
colon prefixed strings in a user specific nqn. Unit tests included.

Create a list of valid properties with get and set callbacks (set is
optional to allow read-only fields).
Remove handling for fields declared as "reserved" in the NVMe over
Fabrics 1.0 specification.
Also simplify the vcprop structure to only contain the required fields.

Previously, we used cap_lo and cap_hi to represent the 32-bit halves of
the full CAP register. However, it is simpler to keep them in a single
64-bit structure, and is no less efficient on 64-bit platforms.
Also name the NSSRS field from NVMe 1.2, which was previously reserved.

NVMe over Fabrics defines its own NVMe Qualified Name (NQN) format; it
does not use iSCSI Qualified Names.
Also change the default node base for nvmf_tgt to "nqn.2016-06.io.spdk".
