per Intel policy to include file commit date using git cmd
below. The policy does not apply to non-Intel (C) notices.
git log --follow -C90% --format=%ad --date default <file> | tail -1
and then pull just the 4 digit year from the result.
Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc). Contribution date used
"--follow -C95%" to get the most accurate date.
Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line. Simply there for
consistency.

Added a new RPC, vmd_rescan, that forces the VMD driver to do a rescan
of all devices behind the VMD. A device that was previously removed via
spdk_vmd_remove_device() will be found again during vmd_rescan.

This doesn't work anyway and can cause creating duplicate bus objects if
vmd_scan_single_bus() is called on a parent bus with previously
allocated child buses. Also, while here, removed a few unused functions
and flags in struct vmd_adapter.

This makes it possible to notify other PCI device providers (VMD) that a
PCI device is no longer used. The VMD will driver will unhook that
device and free any resources tied to it.

Now that we have a attach_device() callback, the devices can be hooked
during spdk_pci_device_attach(). With DPDK, driver->cb_fn() is called
in pci_device_init(), so we need to do the same in
spdk_pci_hook_device().

This allows to free resources tied to a vmd_pci_device that isn't on the
dev_list or wasn't hooked to the PCI driver. Also, use that function
whenever a vmd_pci_device is freed instead of regular free().

Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file. This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.
Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text. There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".
Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

The config_bus_number is an offset within the config space reserved for
the devices behind the VMD, while bus_number refers to the actual bus
number assigned by VMD that depend on the VMCAP and VMCONFIG registers.
So, to access the mapped config space we have to use config_bus_number.
We didn't do that when resetting root ports', which could lead to
segfaults if these values were different, as we'd access unmapped
memory.

None of the current DEBUGLOGs are in any kind of
performance critical code path. Making them INFOLOG
means that we can enable them even in release builds
to get additional information if needed.
(Note: planning to do this with other libraries and
modules as well.)

The root ports might have been configured by some other driver (e.g.
Linux kernel) prior to loading the SPDK one, so we need to clear it. We
need to before the scanning process, as it's depth-first, so when
scanning the initial root ports, the latter ones might still be using
stale configuration. This can lead to two bridges having the same
secondary/subordinate bus configuration, meaning that their config space
would map to the same memory area, which, of course, isn't correct.
This has manifested in issue #2413, where two root ports were configured
to use the same secondary bus. This caused an endpoint device to be
enumerated twice on two different root ports, with the first instance
being broken once the second port was configured by the SPDK driver.

Add the new device ID for VMD devices so VMD devices
can be unbound and used with the SPDK setup script.
Bus numbering for VMD devices is different on IceLake platforms,
and only half of the bus numbers are available. Add a function to
set the starting bus number and the max bus number by reading the
new BUS_RESTRICT_CAP and BUS_RESTRICTIONS VMD registers.

POSIX defines PRId64/PRIu64/PRIx64 for printing 64-bit values in a
portable way. Replace a few references to %ld to remove the assumption
about the size of a long. Similarly, use %z with size_t arguments.
Where the value being printed is an unsigned 64-bit value, use PRIu64
instead of %ld.
Explicitly test for not __linux__ where that is the intent, rather
than testing for __FreeBSD__.
Cast pointer to uintptr_t before aligning it, rather than using
a specific integer size which may not be large enough to store a
pointer.

This patch removes the string from register component.
Removed are all instances in libs or hardcoded in apps.
Starting with this patch literal passed to register,
serves as name for the flag.
All instances of SPDK_LOG_* were replaced with just *
in lowercase.
No actual name change for flags occur in this patch.
Affected are SPDK_LOG_REGISTER_COMPONENT() and
SPDK_*LOG() macros.

You don't get notified when someone starts using your hooked
device, so there's not much gain from knowing when someone
stops.
Remove that callback and also move DPDK device detach under
the same lock which sets the pending_removal flag. This eliminates
a data race window when hotremove notification could arrive
after device was detached, but before it was scheduled to be
removed.
vmd and ioat nest the spdk_pci_device struct and abigail complains
even though the parent structs only have forward declarations in
public headers. Adding those two structs to the suppression list
doesn't help though. Abidiff still complains about the pci device
struct being changed, probably because ioat.h and vmd.h both include
env.h. Abidiff suppresion list should eventually be split per-lib,
but for now ignore struct spdk_pci_device changes globally.

The hooked spdk_pci_device was being removed when someone
stopped using it (detached from it). The following didn't
work:
$ rpc.py bdev_nvme_attach_controller 10000:00:01.0
$ rpc.py bdev_nvme_detach_controller 10000:00:01.0
$ rpc.py bdev_nvme_attach_controller 10000:00:01.0
The second attach can't find PCI device 10000:00:01.0.
To fix it, simply don't do anything in VMD on device detach.
The entire callback will be removed soon. You don't get
notified when someone starts using your hooked device, so
there's not much gain from knowing when someone stops.

The address was padded before the size of requested area was checked,
which for areas bigger than available space resulted in unnecessarily
padding the remaining area.

This patch adds support for hotplug / hotremove detection for devices
behind the VMD. The detection acts similarly to the one implemented for
regular PCIe devices, that is user has to periodically call probe
function. Additionally, for applications not using SPDK's event
framework, spdk_vmd_hotplug_monitor has to be called periodically as
well.

Changed the way the devices are enumerated, so that only the first one
performs active enumeration and stores the result on the config bars of
the devices, while subsequent ones use those saved values. It prepares
the code for hotplug support, as it'll make it easier to detect it.

Moved the base / limit register reset to a separate function. It'll
make it easier to perform the reset only under specific conditions are
met (e.g. only during the initial setup).

This patch adds support for LED management of the devices behind VMD.
It's possible to set four different states: off, on (fault), 1Hz
blinking (rebuild), and 4Hz blinking (identification).

First check both vmd_pci_device for not being a NULL,
same as vmd_adapter structure being present.
Otherwise dev was dereferenced before checking for NULL.

spdk_vmd_init() will attach all VMD devices that were
unbinded from system. There is not need to specify VMD
bdf in VMD public interface since it can be controlled
by setup.sh script.
