per Intel policy to include file commit date using git cmd
below. The policy does not apply to non-Intel (C) notices.
git log --follow -C90% --format=%ad --date default <file> | tail -1
and then pull just the 4 digit year from the result.
Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc). Contribution date used
"--follow -C95%" to get the most accurate date.
Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line. Simply there for
consistency.

In SPDK, declarations have the return type on the same line. Definitions
have the return type on a separate line. Astyle has an option for
enforcing this. Unfortunately, it seems to have two bugs:
1) It doesn't work correctly at all on C++ files.
2) It often fails on functions that return enums, or long type names
Deal with 1) by adjusting the check_format.sh script to only tell astyle
to fix return type line breaks for C files and not C++. Deal with 2) by
adding a few typedefs to work around the problem.

Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file. This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.
Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text. There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".
Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

When a subsystem is being deleted, we disconnect all qpairs
and when the last qpair for some controller is disconnected,
we start controller desctruction process. This process requires
to send a message to subsystem's thread to remove the controller
from the list in the subsystem and after that send a message to
controller's thread to release resources.
The problem is that the subsystem also destroys all attached
controllers. This order is unpredictable and we may get
heap-use-after-free or double free.
To fix this problem we can rely on the fact that the subsystem
can only be destroyed in incative state, that means that all
qpairs linked to the subsystem are already disconnected and
all controllers are already destroyed or in the process of
destruction.
spdk_nvmf_subsystem_destroy API is now can be asyncrhonous,
it accepts a callback with cb argument.

This is the first patch in the patch series to control ANA states not only
as a unit of subsystem listener but also as a unit of ANA group and create
user preferred mapping between namespaces and ANA groups within a single
subsystem.
This patch adds anagrpid to both spdk_nvmf_ns and spdk_nvmf_ns_opts, and adds
ana_group array to spdk_nvmf_subsystem to count number of namespaces per ANA
group within a single subsystem. The size of the ana_group array is equal
with the size of the namespaces.
For each subsystem, allocate ana_group array regardless of the value of
ana_reporting of the subsystem.
For each namespace, at its creation, initialize anagrpid explicitly to be equal
with nsid by default and increments the corresponding entry of the ana_group
array of the subsystem regardless of teh value of the ana_reporting of thee
subsystem.
Hence the contents of the created ANA log page is not changed even if the
algorithm to crete ANA log page is changed.
Additionally this patch adds a unit test case that one ANA group
has multiple namespaces.

The specification says:
"A host may replace its reservation key without regard to its registration
status or current reservation key value by setting the Ignore Existing Key
(IEKEY) bit to '1' in the Reservation Register command."
So for this case we treat it as a new registrant, also add UT to cover
the added cases.

In test_spdk_nvmf_ns_event, when doing ns resize it will call
spdk_for_each_channel() to pause the subsystem. But it doesn't call
spdk_io_device_register() in advance. This is wrong and may run into
assertion with the coming change since the io_device is not found.

1) use spdk_bdev_get_name() accessor
2) use __SPDK_BDEV_MODULE_ONLY #define
The latter allows nvmf to just get the spdk_bdev_module
definitions and APIs that it needs for claiming bdevs
for purposes of avoiding the same namespace used in
different subsystems.
This also ensures that future changes to structures
like spdk_bdev and spdk_bdev_io will not cause
lib/nvmf so version changes.
Note: we include bdev_module.h explicitly in the
nvmf/subsystem unit tests now, before including
subsystem.c, because the unit tests do depend on
knowing the internal structure of spdk_bdev.

Additionally, the user can specify a namespace to also pause during the
operation.
This allows for the management of hosts, listeners, and the addition of
namespaces all while I/O to other namespaces is occurring. Pausing a
specific namespace also allows for the removal of that namespace without
impacting I/O to other namespaces in the subsystem.

It is optional to register i.e. most transports will not need to take
any action. It allows additional verification of ns/bdev capabilities
on transport layer before actual attachment to the subsystem.

With new spdk_nvmf_transport_listen function it should be possible to
add generic options without breaking API/ABI. For now it only delivers
json parameters which can be decoded on a transport specific layer.
This is similar to what was done for spdk_nvmf_transport_create and opts there.

With some build environments (e.g. clang, see #1613) linking fails with
unresolved references. This is caused by the inclusion of 'unused code'
which is usually discarded by the linker. The 'unused code' contains
references to functions that have not been 'stubbed' out. The failure
can be seen by removing 'LDFLAGS += -Wl,--gc-sections' in
spdk.unittest.mk
Resolved by adding stubs for missing references. These are never called
so return an arbtrary default value.

Added asynchronous event notices for discovery log change
as per nvme fabrics spec 1.1. This allows a host with persistent
connection to discovery controller to automatically connect to any
new subsystem available to the host automatically.
According to nvme fabrics spec 1.1, if the connect command specifies a
non-zero keep alive timer value and the discovery controller does not
support asynchronous events then we need to return Connect Invalid.
Since SPDK does not implement this check instead added support
for asynchronous events in discovery controller.

Add an new API spdk_nvmf_subsystem_add_ns_ext() to pass not bdev but
bdev_name to fix the race condition due to the time gap between
spdk_bdev_get_by_name() and spdk_bdev_open(). A pointer to a bdev is
valid only while the bdev is opened.
spdk_bdev_open() has been replaced by spdk_bdev_open_ext() but the
issue still existed.
Update the corresponding unit tests accordingly.
Then replace the internal of spdk_nvmf_subsystem_add_ns() by
spdk_nvmf_subsystem_add_ns_ext() call.

This patch removes the string from register component.
Removed are all instances in libs or hardcoded in apps.
Starting with this patch literal passed to register,
serves as name for the flag.
All instances of SPDK_LOG_* were replaced with just *
in lowercase.
No actual name change for flags occur in this patch.
Affected are SPDK_LOG_REGISTER_COMPONENT() and
SPDK_*LOG() macros.

CUnit provides a helper macro CU_ADD_TEST() that
simplifies usage of CU_add_test() function.

This patch changes the way that unittests initialize.
The new way is shorter and simpler.
It assumes that error during initialization is a fatal error,
but in our cases it always is.
This patch is a followp up to
https://review.spdk.io/gerrit/c/spdk/spdk/+/930
lvol_ut.c is the only test that skipped because
it runs same tests multiple times which is not allowed
by new method.

This was recently made asynchronous to support virtualized transports.
However, we're moving to add a new call to associated a listener with a
subsystem to transports and the operation that needed to be asynchronous
will actually be performed there. For simplicity, make this synchronous
again.

Add file based reservation information definition, the data structure
can be used to store all the reservation information to a json

NVMe bdev module support separate metadata now but NVMf subsystem
cannot process bdev with separate metadata yet.
Hence reject any bdev with separate metadata to be attached
explicitly by this patch.

For reservation feature in NVMoF, we can't support the persist through
power loss feature, now we will add the configuration file parameter
with Namespace, after users set the configuration file parameter with
one NS, then the PTPL feature can be enabled.

Now Host can get an asynchronous event notification when
registrants were unregistered/preempted or reservation was
released from the associate namespace, Host can send
get log page to clear related log pages and reservation
report to get the full overview of current reservation
configuration.
