a6dbe3721e update Intel copyright notices
per Intel policy to include file commit date using git cmd
below.  The policy does not apply to non-Intel (C) notices.

git log --follow -C90% --format=%ad --date default <file> | tail -1

and then pull just the 4 digit year from the result.

Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc).  Contribution date used
"--follow -C95%" to get the most accurate date.

Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line.  Simply there for
consistency.

488570ebd4 Replace most BSD 3-clause license text with SPDX identifier.
Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file.  This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.

Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text.  There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".

Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

1763a126cc test: optimize unit test .gitignore files
Just add a single .gitignore file in test/unit
that covers *_ut.  That allows us to eliminate
100 .gitignore files in the test/unit directory
hierarchy.

8381b98ef4 lib/iscsi: Add portal_group_resume() and add pause parameter to portal_group_open()
Add an new function iscsi_portal_group_resume() and add an new parameter
pause to iscsi_portal_group_open(). They will be used in the following
patches to pause listening portals while target nodes are created at
start-up because Windows hosts do not retry login.

Pausing and resuming portal group is possible because we can unlisten
temporarily by simply not calling accept(). Any inbound connection
requests are queued to the backlog and once the backlog queue is full,
further inbound connection requests are simply dropped. If we restart
calling accept(), we will dequeue the backlog and be ready for more
connecitons.

c70d03f32a test/unit/lib/iscsi: Add unit test stubs
With some build environments (e.g. clang, see #1613) linking fails with
unresolved references. This is caused by the inclusion of 'unused code'
which is usually discarded by the linker. The 'unused code' contains
references to functions that have not been 'stubbed' out. The failure
can be seen by removing 'LDFLAGS += -Wl,--gc-sections' in
spdk.unittest.mk

Resolved by adding stubs for missing references. These are never called
so return an arbtrary default value.

6d86242dc5 subsystem/iscsi: remove legacy config support
This patch removes legacy config support in iscsi subsystem.
All options through the legacy config are already reflected in JSON.

Following RPCs are corresponding to removed legacy config sections:
Initiator groups - iscsi_create_initiator_group and iscsi_initiator_group_*
Subsystem options - iscsi_set_options
Portal groups - iscsi_create_portal_group
Target node - iscsi_create_target_node and iscsi_target_node_*

10d6218444 lib/iscsi: Create portal group as public or private portal group
In SPDK iSCSI target, portal group works almost as identifier of
portal.

To support iSCSI login redirection, we need to have two types of
portal groups, public and private portal groups.

We need portals of public portal groups to redirect to a portal in
a private portal groups at login via temporary login redirection
funciton, and we need to make SendTargets return only portals in
public portal groups.

To do these simply, we mark primary or secondary portal group expicitly
at its creation by this patch.

6917f6e061 lib/iscsi: Remove inclusion of SPDK event library except shared memory ID for iSCSI top application
iSCSI library still needs to include SPDK event library only for
shared memory ID to allocate shared memory for iSCSI top application.

We will move the code to module library separately later but remove
all other inclusion of SPDK event library in this patch.

be1489b9b9 lib/iscsi: Rename iSCSI global data from g_spdk_iscsi to g_iscsi
The following patch will remove the "spdk_" prefix from iSCSI
internal APIs. The iSCSI global data g_spdk_iscsi is also local in
SPDK iSCSI library. Hence rename it by g_iscsi for consistency.

dcf0ca15c9 ut: make use of CUnit macro to reduce duplications
CUnit provides a helper macro CU_ADD_TEST() that
simplifies usage of CU_add_test() function.
Test name no longer needs to be provided,
as it will be inherited from function name.

78b696bca5 ut: simplify error handling at initialization
This patch changes the way that unittests initialize.
The new way is shorter and simpler.
It assumes that error during initialization is a fatal error,
but in our cases it always is.

This patch is a followp up to
https://review.spdk.io/gerrit/c/spdk/spdk/+/930

lvol_ut.c is the only test that skipped because
it runs same tests multiple times which is not allowed
by new method.

aefe3fe23c lib/iscsi: Remove dry run processing from portal config file parsing
Dry run processing in iscsi_parse_portal() now only checks if
portal string is not NULL. Portals are managed not by fixed size
array but linked list now. Hence dry run processing is not needed
itself now.

So remove the dry_run parameter from iscsi_parse_portal() and
the dry run loop in iscsi_parse_portal_grp().

5adeda4807 lib/iscsi: Send message to add connection to poll group even at startup
The next patch will create a SPDK thread for each poll group at
startup. Hence iSCSI configuration management will be done by
different thread from these poll group threads.

Hence send message explicitly to add connection to poll group even
at startup. We can do this for the current master branch.

Remove some code related with SPDK thread framework from unit tests
for iSCSI portal group because thread management is moved to
connection.

fd77140073 lib/iscsi: Remove the code for the deprecated CPU mask
CPU mask parameter was deprecated in v19.10.
If we remove the related code from the portal parser, we will be
able to use the parser for the iSCSI initiator to know the target
portal in iSCSI fuzz testing.  So let's remove the parser and its
test code here.

fb641c4b54 iscsi: Assign connections to poll groups instead of lcores.
This patch binds poll groups to SPDK thread through IO channel and
assigns connections to poll groups instead of cores.

iSCSI subsystem registers iSCSI global object as an IO device, and create
poll groups as context of IO channels of the IO device.

Each portal get and hold portal group on which the corresponding acceptor is
running.

When a connection is constructed, iSCSI subsystem assigns a poll group
to the connection by getting it from the corresponding portal.

When a connection enters full-feature phase, iSCSI subsystem schedules
the connection to a poll group by round-robin.

Then, each connection can know its running SPDK thread directly and can
use SPDK message passing infrastructure instead of SPDK event framework.

By this change, iSCSI connections are binded to SPDK thread, and not
binded to processor core anymore.

Some other changes in this patch are
- core ID is removed from the output of get_iscsi_connections RPC. The
  upcoming patches will change the RPC to use spdk_for_each_channel and can
  access SPDK thread safely, and add SPDK thread ID instead.
- utilize UT multithread framework added by the last patch to test
  iSCSI poll groups by UT.

39e850d17b iscsi: Remove cpumask from portals
Connections will soon be assigned to poll groups, which will be
dynamically moved between CPU cores based on load. It no longer makes
sense to restrict certain portal groups to specific cpu cores in this
model.

e036215fe9 thread: Add a mechanism to exit a lightweight thread
Lightweight threads may now be exited by calling
spdk_thread_exit() within the thread. The framework
polling the thread can release the resources associated
with that lightweight thread by calling
spdk_thread_destroy().

893e02a54f iscsi: Remove the prefix spdk_ from names of private functions
iSCSI library had used the prefix spdk_ for most functions regardless
of private or public. Using the prefix spdk_ only for public functions
will be helpful to distinguish private and public functions, and

Besides in iscsi.c static variable spdk_arc4random_initialized had
the prefix spdk_, and change it to g_arc4random_initialized according
to the SPDK's good practice.

iSCSI library still have some issues but is more stable than before
and now will be the good time to adjust the naming rule to other
libraries.

This patch doesn't change any behavior.

not needed.  This also fixes an issue where touching an included

9c2aea2ad5 build: remove duplicate spdk.app.mk includes
spdk.unittest.mk includes spdk.app.mk, but some unit test
Makefiles include both spdk.unittest.mk and spdk.app.mk,
meaning spdk.app.mk gets included twice.  Fix that.

This hasn't been an issue because spdk.app.mk currently
only includes variables - but no rules.

56d35c5d13 test: forbid use of CU_ASSERT_FATAL
Static analyzers don't see CU_ASSERT_FATAL as truly fatal,
and will complain that code later in the function may try
to dereference a NULL pointer.  So we added SPDK_CU_ASSERT_FATAL
which should be used instead.

This still trips people up sometimes though - the static analyzer
complains and then the developer will add other checks that
pointers are not NULL.

So instead, forbid use of CU_ASSERT_FATAL through check_format.sh
and explain why.  While here, fix up all of the existing CU_ASSERT_FATAL
usages to either CU_ASSERT or SPDK_CU_ASSERT_FATAL.

29c29ff9b6 iscsi: Add JSON config/info dump for portal group.
Support JSON config/info dump for portal group. These are added to
portal_grp.c. Text config dump is moved to portal_grp.c.

Add accesses to g_spdk_iscsi.pg_head are consolidated into
portal_grp.c to extract pg_head from g_spdk_iscsi.

3af8e26df8 iscsi: Change function names of iSCSI subsystem configuration
Portal, portal group, initiator group, and target node don't use fixed size
array anymore. To use the same naming rule in iSCSI subsystem and in SPDK
is helpful to understand and maintain. Current naming rule is not bad but
to use unified and compact name is prioritized at this time.

This patch changes function names as follows:
- spdk_iscsi_init_tgt_nodes() -> spdk_iscsi_parse_tgt_nodes()
- spdk_cf_add_iscsi_tgt_node() -> spdk_iscsi_parse_tgt_node()

- spdk_iscsi_portal_grp_array_create() -> spdk_iscsi_parse_portal_grps()
- spdk_iscsi_portal_grp_create_from_configfile() -> spdk_iscsi_parse_portal_grp()
- spdk_iscsi_portal_create_from_configline() -> spdk_iscsi_parse_portal()
- spdk_iscsi_portal_grp_array_destroy() -> spdk_iscsi_portal_grps_destroy()

- spdk_iscsi_init_grp_array_create() -> spdk_iscsi_parse_init_grps()
- spdk_iscsi_init_grp_create_from_configfile() -> spdk_iscsi_parse_init_grp()
- spdk_iscsi_init_grp_array_destroy() -> spdk_iscsi_init_grps_destroy()

For example "parse" is already used to explain the operation in portal_grp.c.
Hence using "parse" is familiar and not strange in iSCSI.

2ea005f0a9 iscsi: Unify the ordering of PG create_op between conf-file and JSON-RPC
Orderings of portal group create operation are diferrent between
config file and JSON-RPC. Unification is necessary for correct
concurrency control and the global accept poller like NVMf-tgt.
Hence unify the ordering of operations in this patch.

Common ordering of portal group create operation between configuration
file  and JSON-RPC after this patch is the following:
 - create a portal group
 - create portals
 - add the portals to the portal group
 - open the portals of the portal group
 - add the portal group to the global portal group list

After this patch, the gap between listening socket and accepting socket
will be increased a little when portals groups are creted by config file.
However this will cause no issue because of the TCP backlog and resend
mechanism.

Besides, necessary concurrency control is added and minor refactoring
is done.

About portal group delete operation, orderings of application shutdown
and JSON-RPC are already unified.

04c6347b4e iscsi: Consolidate checking uniqueness of PG into register/unregister
Checking uniqueness of portal group is done without mutex and
before register/unregister. This is not thread-safe.

Hence this patch is added to ensure PG uniqueness.

A little related refactoring is also done.

601bcbcf66 util: extend cpumask to hold more than 64 cpus
Fixes github issue #218.

This patch introduces spdk_cpuset object to store and manipulate
the set of individual CPUs. The main objective of this object is
to replace cpumask declared as uint64_t and extend the limitation
of supported CPUs (lcores) above 64 CPUs.

spdk_cpuset is always allocated dynamically and accessed by opaque
pointer, what makes it easier to extend in the future without
breaking API/ABI.

This patch also extends parsing function allowing to set cpumask
using a list of cpus e.g. "[0-4,10,12]" sets mask of 0,1,2,3,4,10,12
as well as hexadecimal string with and without "0x" prefix.

f587197d21 iscsi&iscsi/ut: Unify cpumask decode between iSCSI.conf and JSON-RPC
Currently cpumask cannot be specified for each portal when it is
created by JSON-RPC and portal group creation is not unified
between iSCSI.conf and JSON-RPC.

This patch does the following:
 - cpumask string is decoded in spdk_iscsi_portal_create() which
   is common between iSCSI.conf and JSON-RPC.
 - parsing configline of portal is difficult to understand and
   hence it is refactored.
 - UT code is added.

 JSON-RPC will be added by the next patch.
