Format LBA size (FLBAS) is updated to have:
Bit 3:0 as least significant 4 bits for format index
Bit 6:5 as most significant 2 bits for format index
NVMe format command fields are updated accordingly.
Add a new helper function to fetch the correct format index.
Update examples and unit test files accordingly.

Both the length of a request and the number of ranges to copy are
controlled by the user, so we should check them and return an error
instead of asserting that they're correct.
This fixes the `test/nvmf/target/fabrics_fuzz.sh` test.

Optimal I/O boundary causes I/O to be split in the nvme driver. This is
a problem for writes if write_unit_size > 1 because the split I/O may
not match the write_unit_size.

NVMf target reports copy command support if all bdevs in the subsystem
support copy IO type. Maximum copy size is reported for each namespace
independently in namespace identify data. For now we support just one
source range.
Note, that command support in the controller is initialized once on
controller create. If another namespace which doesn't support copy
command is added to the subsystem later, it will not be reflected in
the controller data structure and will not be communicated to the
initiator. Attempt to execute copy command on such namespace will
fail. This issue is not specific to copy command and applies also to
write zeroes and unmap (dataset management) commands.

per Intel policy to include file commit date using git cmd
below. The policy does not apply to non-Intel (C) notices.
git log --follow -C90% --format=%ad --date default <file> | tail -1
and then pull just the 4 digit year from the result.
Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc). Contribution date used
"--follow -C95%" to get the most accurate date.
Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line. Simply there for
consistency.

When Link Time Optimization is enabled, compiler can sometimes produce
additional warnings saying that some variables may be uninitialized.
To supress the warning it is enough to add explicit initialization
of the variable causing the issue, in this case 'iovcnt = 0'.

When `dif_insert_or_strip` is enabled, NVMf library will do
DIF insert and strip automatically, client isn't aware of
it, when `dif_insert_or_strip` is disabled, we will report
Namespace E2E Protection Capabilities to client, but we
don't process PRACT and PRCHK flags in NVMf library, so
here we don't report the capabilities to client and leave
the use of extended LBA buffer to users.

In SPDK, declarations have the return type on the same line. Definitions
have the return type on a separate line. Astyle has an option for
enforcing this. Unfortunately, it seems to have two bugs:
1) It doesn't work correctly at all on C++ files.
2) It often fails on functions that return enums, or long type names
Deal with 1) by adjusting the check_format.sh script to only tell astyle
to fix return type line breaks for C files and not C++. Deal with 2) by
adding a few typedefs to work around the problem.

Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file. This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.
Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text. There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".
Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

Since spdk_bdev_zcopy_end() cannot really fail (it only fails if we pass
a bad bdev_io), we can simplify the nvmf zcopy_end functions by making
them void and always expect asynchronous completion.

If a request gets resubmitted and is completed immediately (i.e. the
processing function returns SPDK_NVMF_REQUEST_EXEC_STATUS_COMPLETE), the
upper layer needs to be notified via spdk_nvmf_request_complete().

Additionally, the NVMe completion status is now updated and the IOs are
queued if the bdev layer doesn't have enough IO descriptors. It makes
the zcopy operations behave similarly to the other IO operations.

When NVMe passthru command (IO or admin) fails on submission (e.g. it
is not supported), set DNR bit in completion status field. There is no
sense in retrying the command in this case.

the backend iSCSI bdev supports physical_block_size=4K
iSCSI targets exposes physical block size using
logical_block_per_physical_block_exponent in READ_CAPACITY_16
NPWG is one of the way to let Linux nvme driver set
physical_block_size of the nvme block device.
This patch adds spdk_bdev.phys_blocklen which is updated if the iSCSI
backend exposes physical_block_size.
Later phys_blocklen is used in nvmf to set NPWG and NAWUPF to report
back during NS identity.
Linux driver uses min(nawupf, npwg) to set physical_block_size.
Similarly in scsi_bdev fill lbppbe in READ_CAP16 response
based on spdk_bdev.phys_blocklen.

This patch removes the string from register component.
Removed are all instances in libs or hardcoded in apps.
Starting with this patch literal passed to register,
serves as name for the flag.
All instances of SPDK_LOG_* were replaced with just *
in lowercase.
No actual name change for flags occur in this patch.
Affected are SPDK_LOG_REGISTER_COMPONENT() and
SPDK_*LOG() macros.

The new abort functionality doesn't take custom admin cmd
handlers into account.
This commit allows setting a custom admin cmd handler
for abort that provides the ability to influence the
bdev lookup to which the abort is sent to.

This is a public header that needs to be accessible to
code outside of the SPDK project. The spdk_internal/
directory does not end up getting packaged - it's just for
headers used by multiple libraries within SPDK.

For ACWU we always set value 1 because bdev holds
information specific for namespace only. This value
actually does not matter because we also set NACWU
which makes ACWU irrelevant. We set ACWU because
NVMe specs requires ACWU != 0 if fused commands
are supported.

This commit provides the capability to install a
custom admin command handler for NVMF.
It can be used to implement or replace NVMe admin commands that
are currently not handled by the NVMF subsystem.
The handler implementation is pretty generic and the handler function
has to figure out what to do with the command based on the bdevs
that are configured for the subsystem.
In cases where admin commands need to be forwarded to an NVMe bdev,
the commit provides functions that allow access to the underlying bdev.
There is an example handler in lib/nvmf/custom_cmd_hdlr.c.

This change allows setting of the NVMe completion queue
CDW0 in spdk_bdev_io_complete_nvme_status.
Before that change, handling of vendor specific NVMe IO
commands was limited since there wasn't a way to return
command specific info back to the initiator.

When the NVMf controller's flag dif_insert_or_strip is enabled, DIF is
inserted for write I/O and stripped for read I/O, and the corresponding
NVMe-oF initiator should not be aware of the DIF setting of the
backend bdev.
Hence this patch hides the DIF setting of the backend bdev
when the flag dif_insert_or_strip is enabled.

Add a helper function to get necessary DIF information and set
them into the passed DIF context and return. This function will
be called only when the specific requirement is satisfied and
the caller will be added in the next patch.

If users set the persist through power loss configuation file,
that means the Namespace has the capability to support ptpl
feature, here we added a ptpl_activated flag to indicate that
the users enable the feature or not. Users can use Set features
or Reservation Register commands to change the value.
