We callocated the listener, but if we to execute the listening
behaviors which got failed, and all the other resources have been released,
only left listener.
Fixes issue #1326.

This was recently made asynchronous to support virtualized transports.
However, we're moving to add a new call to associated a listener with a
subsystem to transports and the operation that needed to be asynchronous
will actually be performed there. For simplicity, make this synchronous
again.

The resources allocated to a queue pair do not need to be directly
correlated to the queue size requested by the initiator in NVMe-oF, as
long as enough resources are present. The RDMA transport, for instance,
does complex pooling of the resources behind the scenes when using a
shared receive queue.
Simplify the resource allocation for a TCP qpair to just always allocate
the max allowed queue size right away. This is a configurable parameter,
so system administrators can adjust for their needs. The initiator may
then request a queue size less than or equal to that, which will only be
enforced by queue depth counting and not impact the actual number of
resources allocated on the target.
This change relies on the MaxC2HSize being equal to the Maximum Data
Transfer Size (MDTS) reported. That is the default configuration, but
MDTS is configurable. Changing the MDTS with this patch to a value
larger than 128k will cause the target to break. This is addressed in
the next patch in this series.

It can be useful for passing additional information about nvmf
target to a handler for new nvmf connections. Context can be
stored in globals as it is currently done in nvmf code. However
in case of multiple targets or languages where accessing global
state is challenging (i.e. Rust), this becomes inconvenient.

We can merge two loops of req->buffers and req->iov into a single
loop and merge two variables, req->num_buffers and req->iovcnt into
a single variable. For the latter, use req->iovcnt because it is
also used for in-capsule data.

This patch is the end of the effort to unify buffer allocation
among NVMe-oF transports.
This patch aggregates multiple calls of spdk_nvmf_request_get_buffers()
into a single spdk_nvmf_request_get_buffers_multi().
As a side effect, we can move zeroing req->iovcnt into
spdk_nvmf_request_get_buffers() and spdk_nvmf_request_get_buffers_multi()
and do it in this patch.

This patch is close to the end of the effort to unify buffer allocation
among NVMe-oF transports.
Merge each transport's fill_buffers() into common
spdk_nvmf_request_get_buffers() of the generic NVMe-oF transport.
One noticeable change is to set req->data_from_pool to true not in
each specific transport but in the generic transport.
The next patch will add spdk_nvmf_request_get_multi_buffers() for
multi SGL case of RDMA transport.
This relatively long patch series is a preparation to support
zcopy APIs in NVMe-oF target.

The individual transports will adjust these sizes when
necessary. In fact, we have to remove this check, since
RDMA transport may adjust the io_unit_size based on the
max number of SGEs - and can adjust it to a value that
will fail this check if we reload the configuration.

When this structure was brought up to the generic layer, the tcp
transport was using max_io_size and the rdma transport was using
io_unit_size. In the interest of conserving memory, we should use
io_unit_size instead of max_io_size.

For TCP/IP transport, we need to remove the socket
from the polling group since we do not want to keep the
tgroup info in the NVMe/TCP qpair, it should be general.

- Move most of the target opts from nvmf_tgt to nvmf_transport.
- Update transport create functions to pass in transport opts.
- When transport opts are NULL in transport create function, use
target opts. (for backward compatiblity)
- Part 1 of 2 patches. Part 2 (to follow after part 1 accepted)
will allow independent creation of transport with specific opts
while maintaining backward compatibility with current apps and
rpc configuration that still use the add listener method to
create a transport.

In RDMA, qpairs can't be removed from poll groups because
the poll group defines the completion queue. So don't
allow this operation anymore, even if it were theoretically
possible on other transports.

At times, it may be necessary to free requests without completing them.
For example, when freeing a qpair, one needs to free the AER sent from
the host before deleting the qpair. It is important not to send a
completion for the AER because:
1. According to the spec, this will trigger the host to send another AER
2. No Asynchronous Events have occured, so we should not complete the
AER.

Instead of polling each individual qpair, polling is now
done by poll group. This allows transports to use more
efficient polling schemes in the future.
The RDMA transport as of this patch still just loops
over each qpair in the group and polls it individually,
so this patch results in no performance change yet.

For now, this is a name change of controller to poll_group
in the transport layer. Over time, the poll_group will
become a more general concept than a controller, allowing
for qpairs to be spread across cores.

There are now three simple functions on the transport:
listen(transport, trid)
stop_listen(transport, trid)
accept(transport)
This makes the code quite a bit simpler.

one global nvmf target, but this paves the way for multiple.
