a6dbe3721e update Intel copyright notices
per Intel policy to include file commit date using git cmd
below.  The policy does not apply to non-Intel (C) notices.

git log --follow -C90% --format=%ad --date default <file> | tail -1

and then pull just the 4 digit year from the result.

Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc).  Contribution date used
"--follow -C95%" to get the most accurate date.

Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line.  Simply there for
consistency.

8dd1cd2104 check_format: For C files only, fix return type breaks
In SPDK, declarations have the return type on the same line. Definitions
have the return type on a separate line. Astyle has an option for
enforcing this. Unfortunately, it seems to have two bugs:

1) It doesn't work correctly at all on C++ files.
2) It often fails on functions that return enums, or long type names

Deal with 1) by adjusting the check_format.sh script to only tell astyle
to fix return type line breaks for C files and not C++. Deal with 2) by
adding a few typedefs to work around the problem.

488570ebd4 Replace most BSD 3-clause license text with SPDX identifier.
Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file.  This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.

Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text.  There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".

Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

6b7e9d0af2 Lib/iSCSI: add the LUN Resize support
From SAM-4, section 5.13 (Sense Data);
“When a command terminates with a CHECK CONDITION status, sense data shall be returned
in the same I_T_L_Q nexus transaction (see 3.1.50) as the CHECK CONDITION status. After
the sense data is returned, it shall be cleared except when it is associated with a unit
attention condition and the UA_INTLCK_CTRL field in the Control mode page (see SPC-4)
contains 10b or 11b.”

SPDK does not set UA_INTLCK_CTRL to 10b or 11b, so we set the unit attention condition
immediately against a single IO or Admin IO after reporting it via a CHECK CONDITION.
Once the failed IO received at iSCSI initiator side, it will be retried. In the case of
resize operation, if there is no IO from iSCSI initiator side, the unit attention
condition will be delayed to report until the first IO is received at the iSCSI target
side.

Meanwhile, we clear the resizing (newly added) flag on our SCSI LUN structure after
first time we report the resize unit attention condition.

The kernel initiator won’t actually resize the corresponding block device automatically.
It will report a uevent, and then you can set up udev rules to trigger a rescan. SPDK
iSCSI initiator will automatically report the LUN size change.

074f503d27 scsi: Delete mutex from spdk_scsi_globals
This mutex is not used anywhere. After removing mutex from struct
spdk_scsi_globals, struct spdk_scsi_globals is empty. Hence then
remove struct spdk_scsi_globals. We can create struct spdk_scsi_globals
again if it becomes necessary.

db35950a13 mk/unit: link with trace library
Linking each unit test with spdk_trace allows for getting rid of lots of
stub definitions for the trace functions.  The behavior stays the same,
as the trace calls result in no-op anyway because none of the unit test
apps enable tracing.

f4b818434b test/unit/lib/scsi: Add unit test stubs
With some build environments (e.g. clang, see #1613) linking fails with
unresolved references. This is caused by the inclusion of 'unused code'
which is usually discarded by the linker. The 'unused code' contains
references to functions that have not been 'stubbed' out. The failure
can be seen by removing 'LDFLAGS += -Wl,--gc-sections' in
spdk.unittest.mk

Resolved by adding stubs for missing references. These are never called
so return an arbtrary default value.

2172c432cf log: simplify SPDK_LOG_REGISTER_COMPONENT
This patch removes the string from register component.
Removed are all instances in libs or hardcoded in apps.

Starting with this patch literal passed to register,
serves as name for the flag.

All instances of SPDK_LOG_* were replaced with just *
in lowercase.
No actual name change for flags occur in this patch.

Affected are SPDK_LOG_REGISTER_COMPONENT() and
SPDK_*LOG() macros.

ab808d2b68 lib/scsi: Pass bdev_name instead of bdev to scsi_lun_construct()
Pass not bdev but bdev_name to scsi_lun_construct() to fix the
race condition due to the time gap between spdk_bdev_get_by_name()
and spdk_bdev_open(). A pointer to a bdev is valid only while the
bdev is opened.

spdk_bdev_open() has been replaced recently by spdk_bdev_open_ext(),
but the issue still existed.

5029fe14b6 scsi: add bdev resize callback support
Currently, the scsi bdev only supports the hotremove event,
and the scsi library uses the deprecated `spdk_bdev_open` function.

In this patch, add the resize event support, so the upper layer
could do more actions, like vhost-scsi could notify the guest os.

For the scsi compatibility, add _ext suffix for some public api.

6b6a3ff91f scsi: add SPC2 RESERVE/RELEASE support
Some OS(Windows and VMWARE esxi) will send SPC2 RESERVE/RELEASE
commands when formatting the filesystems, but in our previous
implementation of reservation feature, we didn't add such support,
in specification SPC3/4, they all include one section "Exceptions to
SPC-2 RESERVE and RELEASE behavior" feature for compatible support of
SCP2 RESERVE/RELEASE, so we add this support now.

Fix issue #1414.

799e0a84af lib/scsi: Merge append and execute SCSI task into a single function
Both for SCSI IO task and management task, append and execute
operations bad bbeen separated into different functions.

Append operation was for LUN reset, and separating into two different
functions was for clarification and readability.

LUN reset is sufficiently stable now.

Merging append and execute SCSI task into a single function is good
as API and enables us to do optimization.

For SCSI management task, merge spdk_scsi_lun_append_mgmt_task into
spdk_scsi_lun_execute_mgmt_task() simply.

For SCSI IO task, merge spdk_scsi_lun_append_task into
spdk_scsi_lun_execute_task() and do a small optimization.
The refined spdk_scsi_lun_execute_task() adds the IO task to the
pending list if there is any pending management task, executes all
existing penging IO tasks first and then the IO task if there is any
pending IO task, or executes the IO task directly otherwise.

Update unit test accordingly.

2144f0b4c3 lib/scsi: Separate pending task and outstanding (submitted) task into different functions
This is a preparation to the following patches to optimize task
submission and remove the spdk_prefix from internal or private
functions.

To avoid using double underscores as the prefix of the function name,
separate pending task check and outstanding task check into
different functions.

78b696bca5 ut: simplify error handling at initialization
This patch changes the way that unittests initialize.
The new way is shorter and simpler.
It assumes that error during initialization is a fatal error,
but in our cases it always is.

This patch is a followp up to
https://review.spdk.io/gerrit/c/spdk/spdk/+/930

lvol_ut.c is the only test that skipped because
it runs same tests multiple times which is not allowed
by new method.

To avoid the starnge formatting, typedef has been used. But this comment
is hard to get the meaning. So stop breaking after return type for this case.

8287ae7814 lib/scsi: Abort management tasks being queued when removing LUN
SCSI management task is for SCSI tasks which is submitted after it,
and all such SCSI tasks are aborted by the last patch when we
remove LUN dynamically. Hence we can abort all SCSI management tasks
being queued when removing LUN.

Add simple unit tests too.

f9583f2c0d lib/scsi: Check pending tasks for the LUN only from the specific initiator
Refine helper functions spdk_scsi_lun_has_pending_mgmt_tasks() and
spdk_scsi_lun_has_pending_tasks() to be able to check tasks only from the
specific initiator.

SCSI port is used by passing the pointer and so simple pointer
comparison is appropriate in the functions.

Add UT code to test the updated functions.

The next patch will change spdk_scsi_dev_has_pending_tasks() to
get not only SCSI device but also initiator port and make iSCSI
target use the function to fix the issue.

8c0c8e5333 scsi: add persistent reservation SCSI commands check
All the SCSI commands have two states: allowed and conflict,
based on different reservation types and I_T nexus, the SCSI
module will check each commands before sending to the backend
device.

7d4684524b test: use test_env in scsi unit tests
Also remove some unneeded stubs since the thread
library is linked by default to all unit tests.

Note that the lun.c unit tests include test_env via
the new include of ut_multithread.c.  It needs ut_multithread
to simplify allocation of threads needed so that the lun.c
code can safely register pollers.

728972cdf3 scsi: Set TMF code prior to calling spdk_scsi_dev_queue_mgmt_task
By subsequent patches for iSCSI, spdk_scsi_dev_queue_mgmt_task()
will not be called directly from the function that knows TMF code,
and currently setting TMF code to SCSI task is done in
spdk_scsi_dev_queue_mgmt_task().

Hence after subsequent patches for iSCSI, to hand off TMF code to
SCSI task, any dynamic context will be required.

To avoid the dynamic context, extract setting TMF code from
spdk_scsi_dev_queue_mgmt_task() and put appropriate place for
each call of spdk_scsi_dev_queue_mgmt_task().

Additionally, in spdk_abort_transfer_task_in_task_mgmt_resp(),
ref_task_tag is got from PDU but getting it from SCSI task is
much easier. Hence get ref_task_tag from SCSI task in the callback.

d0e3f624f0 scsi: LUN reset waits for completion of all prior tasks
IO submissions to backends have to complete first in LUN reset.
Previous patch makes the waiting time limited.

LUN reset don't use timeout for now because even if LUN reset reports
timeout, error handling of the initiator will be escalated and the
initiator will issue stronger command and will wait eventually
until all outstanding tasks completed.

Besides, not using timeout in LUN reset will make implementation simple.

If timeout becomes necessary, I'll do it in the separate patch later.

Hence revert the removal in this patch.

And using the submission queue, suspend IO task submission during

6dd09113e5 scsi: Serialize LUN reset execution
Task management functions don't require performance. Serializing
execution of task managment functions will sipmlify and stabilize
the logic and will be helpful for upcoming patches to support
other task management functions.

This patch introduces two queues, pending and submitted queue,
and serializes LUN reset exection and makes LUN hot removal
wait for LUN reset.

Besides, checking if LUN is NULL is moved to the upper layer as
same as IO task submission.

There was a plan a long time ago to try to output all
unit test results in JSON format for post-processing.
It is used by only one unit test file currently, but the
results aren't used and there are no plans to use it in

f2554ee973 scsi: Change spdk_scsi_task_alloc/free_data() from public to private
This patch does a little cleanup of SPDK SCSI layer APIs.

spdk_scsi_task_alloc_data() is only used in spdk_scsi_task_scatter_data().
spck_scsi_task_free_data() is only used in spdk_scsi_task_put().

The latter was called in UT code but this can be removed without any
degradation.

SPDK SCSI layer is relatively matured and these will not be used
out of lib/scsi/task.c.

Additionally memory leak detected by ASAN is fixed in this patch.

85a61ea6f0 scsi: fix LUN removal/hotremoval path
bdev hotremove event is send directly to hotremove callback given
during spdk_scsi_dev_construct() call. So any bdev hotremove might be
promoted to whole SCSI device removal (like in vhost) wich will trigger
LUN removal. But after returning from hotremove callback LUN and device
might be still referenced (eg to register poller or by outstanding IO).

Even worse: spdk_scsi_dev object is not dynamicaly allocated but
returned from static array which mean there is no way to detect
use-after-free error on spdk_scsi_dev by any tool. This might lead to
using SCSI device that is freed or assigned to different device.

To fix this:
- always delete LUN using hotremove path
- defer spdk_scsi_dev delete/removal after all LUNS are really
deleted.

583e969922 scsi: remove lun name
There is no need to keep a lun name anymore - we always
use the bdev name as the lun name so it is not providing any
additional value. This also keeps us from associating
the same bdev with different LUNs on different iSCSI target
nodes or vhost-scsi controllers.

Side effect of this change is:

1) Use "bdev_name" across the APIs to make it more clear
   what these names refer to.

fc03485b9d scsi/lun: fix hotremove with no io_channels open
When LUN with no io_channels is being hotremoved,
the hotremove poller was being started on the core
pointed by lun->lcore. However, this field is only
valid when there are io_channels open. For example,
when user tries to delete a bdev that's attached to
an unused vhost controller (no io_channels ever opened)
the scsi layer could start a hotremove poller on
lun->lcore == 0. If SPDK runs on a different cpuset -
not containing CPU0 - SPDK could even segfault.

59970a89be astyle: enforce braces around single-line statements
Require braces around all conditional statements, e.g.:

    if (cond)
        statement();

becomes:

    if (cond) {
        statement();
    }

This is the style used through most of the SPDK code, but several
exceptions crept in over time.  Add the astyle option to make sure we
are consistent.

e44731d659 scsi: Remove the claimed flag from LUN
Struct spdk_scsi_lun has the claimed flag as a member.

However the claimed flag is no longer required.

Moreover the claimed flag itself is not used but
spdk_scsi_lun_claim() and _unclaim() functions are used in the
UT code by hard to understand manner. Moving the logic into
spdk_scsi_lun_construct() and _destruct() in the UT code will
be more natural.

Hence remove the claimed flag from LUN and do a little refactoring
in the UT code.
