This patch fixes two issue causing segfault on r2t:
1. pdu buffer is allocated from immediate_data_pool, but data_buf_len is set as data_out_pool
2. task->desired_data_transfer_length is rewrite by iscsi_send_r2t, which causes a wrong calculated pdu->data_buf_len

per Intel policy to include file commit date using git cmd
below. The policy does not apply to non-Intel (C) notices.
git log --follow -C90% --format=%ad --date default <file> | tail -1
and then pull just the 4 digit year from the result.
Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc). Contribution date used
"--follow -C95%" to get the most accurate date.
Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line. Simply there for
consistency.

Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file. This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.
Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text. There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".
Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

The recent changes merged multiple Data-OUT PDUs within the same
sequence into a single subtask up to 64KB.
However, they were not enough.
For a large write operation, the hardware iSCSI HBA host sent an immediate
data whose size was not block size multiples and then more solicit
data through R2T exchanges.
One example for a 64KB write operation was as follows:
host sent SCSI Write with 5792 bytes and F = 1
target replied a R2T
host sent Data-OUT with 15880 bytes
host sent Data-OUT with 11536 bytes
host sent Data-OUT with 2848 bytes
host sent Data-OUT with 11536 bytes
host sent Data-OUT with 5744 bytes
host sent Data-OUT with 12200 bytes and F = 1
The hardware iSCSI HBA host can decide the size of the unsolicited data
but the SPDK iSCSI target can require the host to send the solicited data
whose size is block size multiples.
Hence we merge immediate data to the following R2T data if the immediate
data is not more than 64KB and more R2T data come.
Add another test case to check if the fix works for the above example.

When data segment size is 64KB and data digest is enabled, if
data segment and data digest are split into different two packets,
- pdu->mobj[0] became full first when reading data semgment,
- pdu->mobj[1] was allocated but unused and data digest was read.
In this case, two SCSI write tasks were submitted by mistake and
the second SCSI write task had no data.
Fix the bug in this patch.
When iscsi_pdu_payload_read() is called and pdu->mobj[0] is full,
allocate pdu->mobj[1] only if any of data segment remains to read.

Change a fixed size array to a linked list to manage LUNs per SCSI
device.
Keep the linked list sorted by LUN ID because this is necessary to
efficiently find the lowest free LUN ID or check the specified LUN is free.
To avoid traversing the linked list twice, change scsi_dev_find_free_lun()
to return the LUN which comes just before where we want to insert an new LUN.
Additionally, previously spdk_scsi_dev_add_lun_ext() had not checked if
the specified LUN ID was duplicated. Fix the bug in this patch.
Add unit test cases for the function scsi_dev_find_free_lun().
These changes will enable the following patches to increase
SPDK_SCSI_DEV_MAX_LUN from 64 to 256 without consuming additional memory.

Test scenario is as follows.
Some iSCSI initiator sends a Data-OUT PDU sequence such that the size of
the data segment of any Data-OUT PDU is not block size multiples.
Test if such complex Data-OUT PDU sequence is processed correctly.
Desired Data Transfer Length is 5 * SPDK_ISCSI_MAX_RECV_DATA_SEGMENT_LENGTH / 2.
Number of Data-OUT PDUs is 4. Length of the data segment of the first two PDUs are
SPDK_ISCSI_MAX_RECV_DATA_SEGMENT_LENGTH / 2 - 4. Length of the data segment of the
third PDU is SPDK_ISCSI_MAX_RECV_DATA_SEGMENT_LENGTH. Length of the data segment
of the final PDU is SPDK_ISCSI_MAX_RECV_DATA_SEGMENT_LENGTH / 2 + 8.
Three data buffers should be used and three subtasks should be created and submitted.
The test scenario assume that a iscsi_conn_read_data() call could read
the required length of the data and all read lengths are 4 bytes multiples.
The latter is to verify data is copied to the correct offset by using data patterns.

Test sharing data buffer among multiple Data-OUT PDUs.
Allocate real data buffers and the iscsi_conn_read_data() stub writes
data patterns into them.
The data pattern is to write offset per 4 bytes. This requires read
length to 4 bytes multiples but the requirement will be acceptable.

The following patches will change the handler for Data-OUT PDU to
submit subtask only when 64KB data is read or F bit is set.
Previously, we had created a subtask when processing header and
before reading data segment. Creating a subtask beforehand is not
convenient for the following changes.
Hence create a subtask after reading data segment.
If LUN is removed while processing the Data-OUT PDU, the corresponding
primary task will be terminated by iscsi_clear_all_transfer_task(),
and any subtask completion is not sent to initiator. Hence we can
reject the received Data-OUT PDU safely.

The spec does not disallow TEXT PDUs with no data. In that
case, just return immediately from iscsi_parse_params.
This avoids a NULL pointer dereference with a TEXT PDU that has
no data, but CONTINUE flag is set.

According to iSCSI 7143 spec 11.15.1 (3720 spec 10.15.1),
Rsph->response should be 0 if connection or session closed successfully.
The current code mistakenly compared cid provided by initiator with
inner conn->id parameter, which does not follow the spec.

POSIX defines PRId64/PRIu64/PRIx64 for printing 64-bit values in a
portable way. Replace a few references to %ld to remove the assumption
about the size of a long. Similarly, use %z with size_t arguments.
Where the value being printed is an unsigned 64-bit value, use PRIu64
instead of %ld.
Add a cast when comparing an enum to an integer value.
Add an explicit uint64_t cast to the definition of BLOCK_CNT to prevent
value truncation.

By the recent refactoring, we have no static size array for outstanding
R2Ts per connection. It looks that we do not have any critical reason
to prohibit us from making max outstanding R2Ts per connection configurable.
There are some use cases to use large write I/O intensively (e.g. 128KB).
Let such use cases change the value of max R2Ts per connection by their
responsibility to do performance tuning.
Maximum outstanding R2Ts per task are defined both for iSCSI target
and NVMe-TCP target but maximum outstanding R2Ts per connection is
unique for iSCSI target.
The next patch will add the corresponding iSCSI option.

If the portal group map of the target has a redirect portal,
iscsi_tgt_node_is_moved() fills the buffer by the redirected address
and returns true.
iscsi_op_login_check_target() calls iscsi_tgt_node_is_redirected() before
calling iscsi_tgt_node_access() because login redirection can be
checked before any or after all security check.
If iscsi_tgt_node_is_redirected() returns true, notify login redirection
to the corresponding initiator.

We have no particular requirement to keep both conn->outstanding_r2t_tasks
array and conn->active_r2t_tasks list now.
To improve readability and maintaineability, unify two into the latter,
conn->outstanding_r2t_tasks list. Update unit test accordingly.

Previously iscsi_del_transfer_task() dequeued the task only from
the array conn->outstanding_r2t_tasks[].
process_non_read_task_completion() had dequeued the task from
the tailq conn->active_r2t_tasks then.
However abort_transfer_task_in_task_mgmt_resp had not dequeued the
task from the tailq conn->active_r2t_tasks then.
This was an apparent bug, and is fixed here. Update unit tests
accordingly.

CUnit provides a helper macro CU_ADD_TEST() that
simplifies usage of CU_add_test() function.
Test name no longer needs to be provided,
as it will be inherited from function name.

This patch changes the way that unittests initialize.
The new way is shorter and simpler.
It assumes that error during initialization is a fatal error,
but in our cases it always is.
This patch is a followp up to
https://review.spdk.io/gerrit/c/spdk/spdk/+/930
lvol_ut.c is the only test that skipped because
it runs same tests multiple times which is not allowed
by new method.

Operations to queue iSCSI tasks are in iscsi.c and conn.c and cross
references due to this separation makes us difficult to create unit
tests.
This and subsequent patches will try to disentangle cross references
by moving some functions from iscsi.c to conn.c.
This patch moves spdk_iscsi_conn_handle_queued_datain_tasks() from
iscsi.c to conn.c. For unit tests, we don't add anything new in
this patch and just create necessary simple stubs. After code
movement, new unit tests will be added.

We changed read I/O submission to stop splitting when detecting LUN
hotplug very recently. We can do the same refinement for read I/O
abortion.
In _iscsi_conn_abort_queued_datain_task(), set all remaining length
to the new task and complete it immediately. We keep the code to
process the case that queued_datain_task completed but is still in
queue, but we can change its if condition to assert.
Simplify the corresponding unit tests accordingly, and set
task->scsi.transfer_len in abort_queued_datain_tasks_test() to
exercise the changed paths.
In iscsi_pdu_payload_po_scsi_read(), if task->scsi.transfer_len is not
larger than SPDK_BDEV_LARGE_BUF_MAX_SIZE, no minimum calculation is
necessary and we can substitute task->scsi.transfer_len to
task->scsi.length simply. This change is too small to be an independent
patch and is done together.

Purpose: Do not let the primary task do the I/O if it is
splitted into subtasks. This will make the code simplier,
when all the sub I/Os are finished, we can free the
primary task.
Update the corresponding unit test also.
As a result of this change, when read I/O is split into subtasks,
the primary task uses only some of its data. Hence separate
iscsi_pdu_payload_op_scsi_read() into split and non-split
case explicitly.

By the recent refactoring, SCSI task is configured when getting
DIF context from SCSI layer. Passing not CDB and offset separately
but SCSI task to SCSI layer is more concise and do in this patch.
In iscsi_send_datain(), we have to update task->scsi.offset for the
case that data is split into a sequence, but the update is no harm
because task has completed what it must to do.

We have separated PDU header handler and payload handler, and have
PDU header handlers for each PDU type now.
By using this refinement, we can remove an aggregated helper function
spdk_iscsi_get_dif_ctx() and embed spdk_scsi_lun_get_dif_ctx() into
each PDU header handler.

Add unit tests for PDU header handling except for SNACK Request PDU
because the caes that Error Recovery Level is not matured yet.
The purpose is mainly for increasing coverage, and most were reviewed
separately, and so merge multiple PDU types into a single patch.

Then the original iscsi_execute() is renamed to iscsi_pdu_payload_handle().

Move spdk_iscsi_execute() from iscsi_conn_handle_incoming_pdus()
to spdk_iscsi_read_pdu() and then strip the prefix spdk_ from
spdk_iscsi_execute() and make it private.
This is to introduce state machine into receive incoming PDU processing.

conn->target is initialized to NULL in iscsi_op_login_phase_none
and then configured in iscsi_op_login_session_normal. Hence
subsequent references to target node can be done by conn->target and
related local variables can be removed.

This bug was found by code inspection. When PDU read restarts
after data segment, iov_offset must be reduced by data length.
However iov_offset had been reduced by buffer length by mistake.
Lack of UT code was the main reason of this bug. Hence add UT
code to test the fix of the bug together in this patch.

In a test case of op_login_session_normal_test(), it is expected that

the tag of the session was not initialized.
