This adds a log message indicating that an lvstore is being unloaded due
to its base device being hot removed.

per Intel policy to include file commit date using git cmd
below. The policy does not apply to non-Intel (C) notices.
git log --follow -C90% --format=%ad --date default <file> | tail -1
and then pull just the 4 digit year from the result.
Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc). Contribution date used
"--follow -C95%" to get the most accurate date.
Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line. Simply there for
consistency.

A new parameter io_drain_timeout has been added to spdk_bdev
structure. If this value is unset, the bdev reset behavior
does not change.
The io_drain_timeout controls how long a bdev reset must wait for IO
to complete prior to issuing a reset to the underlying device.
If there is no outstanding IO at the end of that period, the reset
is skipped.

The bdev_lvol_grow_lvstore will grow the lvstore size if the undering
bdev size is increased. It invokes spdk_bs_grow internally. The
spdk_bs_grow will extend the used_clusters bitmap. If there is no
enough space resereved for the used_clusters bitmap, the api will
fail. The reserved space was calculated according to the num_md_pages
at blobstore creating time.

Reserve space for used_cluster bitmap. The reserved space is calculated
according to the num_md_pages. The reserved space would be used when
the blobstore is extended in the future.
Add the num_md_pages_per_cluster_ratio parameter to the
bdev_lvol_create_lvstore API. Then calculate the num_md_pages
according to the num_md_pages_per_cluster_ratio and bdev total size, then
pass the num_md_pages to the blobstore.

Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file. This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.
Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text. There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".
Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

The new blobstore ext API is used when the user
provides ext_io_opts in bdev layer.
To store blobstore ext_io_opts, vbdev_lvol reports
non-zero get_ctx_size in bdev module interface.

This patch makes use of async_fini_start flag to
make fini_start asynchronous.
During this time all lvol stores which have no open
lvols are unloaded. This is required, since lvs
holds claim on the underlying bdev.

Once bdev finish starts, bdev unregister is called on all
unclaimed bdevs. This means that for lvs with at least one
lvol present, there will be a corresponding bdev unregister.
Yet the vbdev_lvol module does not attempt to unload the lvs,
once last lvol from that lvs is unregistered. Leaving
the base bdev for lvs claimed.
This patch fixes that by using fini_start callback from
bdev_module to mark when shutdown begins. After that
last lvol unregistered on lvs will unload it.
Expanded struct lvol_bdev to contain lvol_store_bdev.
Closing the lvol will free spdk_lvol, so lvol->lvol_store cannot
be accessed.
Changed ut_lvol_destroy UT to ut_bdev_finish. Previous UT didn't
really test vbdev_lvol_destroy, but 'hotremove' of the lvol bdev.
In effect there is no hotremove of the lvol bdevs (only lvs bdev).
spdk_bdev_unregister() can only be called from within vbdev_lvol,
or during bdev module finish.
This UT will now check the bdev module finish.
Note that at this point lvs with no lvols will not trigger
lvs unload. Next patches in series will introduce async fini_start,
to allow for the unload.

Use the macros for red black tree provided by Free BSD to speed up bdev
name lookup in spdk_bdev_get_by_name().
In the bdev_multi_allocation test, we can get 3x ~ 5x speed up when
creating multiple bdevs for various bdev nums.

This patch removes the string from register component.
Removed are all instances in libs or hardcoded in apps.
Starting with this patch literal passed to register,
serves as name for the flag.
All instances of SPDK_LOG_* were replaced with just *
in lowercase.
No actual name change for flags occur in this patch.
Affected are SPDK_LOG_REGISTER_COMPONENT() and
SPDK_*LOG() macros.

and changing it on callback.
Since it is based on blob bserrno.
lvol_task is removed in next patch.
This change was motivated by lvol_op_comp() accessing
the task->status pointer on hot path for I/O.
There is no need to do so, so just pass status.
