Separate out the RDMA connection from the
NVMf connection. For now, the RDMA connection
is just embedded in the NVMf connection, but
eventually they will have different lifetimes.

Only move nvmf_request definitions from nvmf_internal.h
for now. Subsequent patches will move more.

The nvmf_request structure holds the pair of pointers
for rx_desc and tx_desc.
