64a268e2e1 iscsi: Fix double dequeue of the primary write task in error handling
By running random, 64KB, 70% write, and 30% read workload and periodical
TCP disconnection from the initiator, segmentation fault have occurred.

By investigation, when the last write task completed and dequeued
the primary write task in spdk_iscsi_task_cpl(), the primary write task
was already dequeued in spdk_clear_all_transfer_task().

This patch dequeues all primary write tasks after all outstanding
tasks complete.

After starting to exit the connection, the connection will not receive
any command. Hence waiting for completion of all pending tasks first will
avoid double dequeue safely.

855633a597 iscsi: Get LUN ID from task instead of Data-Out PDU to fix SCSI compliance test failure
A major SCSI compliance test expects that the LUN field is reserved
(not to be checked) in the SCSI Data-Out PDU.

In that test, the LUN field in the first SCSI Command PDU is 0 and
the LUN field in the SCSI Data-Out PDU sent in response to the R2T
is 0xFFFFFFFFFFFFFFFF. The TTT field in the SCSI Data-Out PDU is 0xF that
is copied from the R2T. 0xF is valid as TTT.

Hence spdk_iscsi_op_data() returns Check Condition because LUN is
not found.

This patch changes to get LUN ID from not the LUN field in the SCSI
Data-Out PDU but the saved task.

The following is excerpts from iSCSI specification (RFC7143). The
behavior of the SCSI compliance test looks against the specification
but changing SPDK will be safer.

11.7.4. Target Transfer Tag and LUN

On outgoing data, the Target Transfer Tag is provided to the target
if the transfer is honoring an R2T. In this case, the Target
Transfer Tag field is a replica of the Target Transfer Tag provided
   with the R2T.

On incoming data, the Target Transfer Tag and LUN MUST be provided by
the target if the A bit is set to 1; otherwise, they are reserved.
The Target Transfer Tag and LUN are copied by the initiator into the
SNACK of type DataACK that it issues as a result of receiving a SCSI
Data-In PDU with the A bit set to 1.

The Target Transfer Tag values are not specified by this protocol,
except that the value 0xffffffff is reserved and means that the
Target Transfer Tag is not supplied. If the Target Transfer Tag is
provided, then the LUN field MUST hold a valid value and be
consistent with whatever was specified with the command; otherwise,
   the LUN field is reserved.

0bb86d1cf3 iscsi: Fix the bug that mutual chap is not enabled by .ini config file
As long as source code and sample config file are checked, mutual
chap for discovery sessions should be enabled by either "Mutual"
or "CHAP Mutual". Especially SPDK iSCSI guides users to use not
"Mutual" but "CHAP Mutual".

The reason is that .INI config dump outputs "DiscoveryAuthMethod CHAP Mutual"
when g_spdk_iscsi.mutual_chap is true.

However "DiscoveryAuthMethod CHAP Mutual" doesn't work as expected.
When it is specified, g_spdk_iscsi.require_chap becomes true but
g_spdk_iscsi.mutual_chap becomes false.

When either "DiscoveryAuthMethod Mutual" or "DiscoveryAuthMethod Mutual CHAP"
is specified, both g_spdk_iscsi.require_chap and g_spdk_iscsi.mutual_chap
become true as expected. But any user cannot guess "Mutual CHAP".

On the other hand, CHAP feature for iSCSI target nodes works as
expected.

Fix this issue according to the implementation in iSCSI target nodes.

One difference between iSCSI target node and discovery service remains
that iSCSI target node doesn't allow "AuthMethod Mutual" but discovery
service allows "DiscoveryAuthMethod Mutual" to be compatible.

b6ee6f9241 iscsi, nvmf, vhost: remove default config file pathnames
Config file pathnames have been possible on the command line
for a very long time, and all of the SPDK testing infrastructure

pulled in by the default pathname instead.

So remove the default pathnames.

ad323b8064 iscsi&scrpts/rpc: Require to specify CHAP secret file explicitly to load it
Previous patches enabled users to configure CHAP secrets dynamically

Loading CHAP secret file is done by default and this will conflict to
JSON config file.

Hence the path to CHAP secret file is required to specify in the config

Users who have used CHAP secret file are expected to specify it explicitly
and this will be no harm for them.

Besides, CHAP secret file is not oly for discovery sessions but also for
login to iSCSI targets. However there were wrong description to make user
misunderstand. Hence remove these wrong description in this patch too.

b6ab3116fc iscsi: Load CHAP secrets from file once at boot and use them in memory
For secure iSCSI targets, dynamic reconfiguration of CHAP secrets is
must to have.

Currently CHAP secrets are loaded for every CHAP authentication
operation. The current implementation will not work correctly
when CHAP secrets in the file are changed dynamically.

If SPDK loads CHAP secrets from the file only at boot and they can be
configured by RPCs, user can change CHAP secrets safely during run time.

Even if there are any users who expect dynamic reconfiguration of
CHAP secrets based on the current implementation, if we provide
this better alternative based on RPCs, they will be able to continue to
satisfy their requirement.

This patch change the current implementation so that SPDK loads
CHAP secrets from the file once at boot and uses them in memory
hereafter.

Besides, use fixed size buffers to hold CHAP secrets.
Previously dynamically allocated buffers by strdup() had been used,
but it required many nomem checks and should be avoided.
Other iSCSI targets/initiators have used fixed size buffers and SPDK
follows others.

Set the size of buffers for both user names and secrets to 256 (the
last byte is for NULL termination). 256 is sufficiently large
compared with others.

CHANGELOG will be updated in the separate patch because new RPCs will
be added instead.

476b1b0fc9 lib/iscsi: fix the queued_datain_task hanlding issue.
Due to the network issue, spdk_iscsi_conn_free_pdu in
spdk_iscsi_conn_flush_pdus_internal will not be executed.
So consider the pdu free, we should call spdk_iscsi_conn_free_pdu
in spdk_iscsi_conn_free_tasks.

Actually, for the task which in queued_datain_task, we have
the following case:

1 The task is not sent to the scsi layer: it means that
the task is not freed, so we should call spdk_iscsi_task_put
here.
2 The task is sent to the scsi layer, but no subtasks
are sent to the scsi layer: It means that the call
back function (spdk_iscsi_task_cpl) will be called,
but since it will have the subtask, so spdk_iscsi_task_put
will not be called, thus, we should call spdk_iscsi_task_put
here.
3 The task is sent to the scsi layer, and some subtasks
are also sent to the scsi layer: It also mean that the
spdk_iscsi_task_put will not be called in spdk_iscsi_task_cpl,
and not all the subtasks will be finished, so the father task
will not be freed, so we should still call spdk_iscsi_task_put
here.

4 The task is sent to the scsi layer, and all the subtasks
are also sent to the scsi layer,
thus this task is not in the queued_data_in_task.

So according to 1-4, we should call spdk_iscsi_task_put here,
and also decrease the data_in_cnt;

a02ab95ebb iscsi: Bug fix for residual count when length of data transfer from SCSI was 0
This bug was caused by the following two changes:

iscsi: Remove duplication of the variable for write completion to bdev
https://review.gerrithub.io/#/c/393582/

iscsi: restore data_transferred accumulation for read
https://review.gerrithub.io/#/c/393713/

For write, bytes_completed is always equal to data_transferred. However,
for read, bytes_completed is based on expected data transfer length and
data_transferred is actual read size. Hence bytes_completed cannot be
used to calculate residual counts.

One of the reason why this bug cannot be found was that there was not any
test case when task->scsi.data_transferred is 0 and task->scsi.length is
not 0. Hence UT code is also added.

Same bug will occur for read sense data. Hence UT code for read sense data
is added in the next patch.
