per Intel policy to include file commit date using git cmd
below. The policy does not apply to non-Intel (C) notices.
git log --follow -C90% --format=%ad --date default <file> | tail -1
and then pull just the 4 digit year from the result.
Intel copyrights were not added to files where Intel either had
no contribution ot the contribution lacked substance (ie license
header updates, formatting changes, etc). Contribution date used
"--follow -C95%" to get the most accurate date.
Note that several files in this patch didn't end the license/(c)
block with a blank comment line so these were added as the vast
majority of files do have this last blank line. Simply there for
consistency.

Many open source projects have moved to using SPDX identifiers
to specify license information, reducing the amount of
boilerplate code in every source file. This patch replaces
the bulk of SPDK .c, .cpp and Makefiles with the BSD-3-Clause
identifier.
Almost all of these files share the exact same license text,
and this patch only modifies the files that contain the
most common license text. There can be slight variations
because the third clause contains company names - most say
"Intel Corporation", but there are instances for Nvidia,
Samsung, Eideticom and even "the copyright holder".
Used a bash script to automate replacement of the license text
with SPDX identifier which is checked into scripts/spdx.sh.

Previously we decided which LUN format is used by the macro constant
SPDK_SCSI_DEV_MAX_LUN. However, as long as we read SAM, the LUN format
can be decided per LUN ID.
Linux host SCSI driver supports 256 LUNs per SCSI device at the
maximum. So we cannot test this fix on any actual system but we
apply this fix for the potential future cases.

This mutex is not used anywhere. After removing mutex from struct
spdk_scsi_globals, struct spdk_scsi_globals is empty. Hence then
remove struct spdk_scsi_globals. We can create struct spdk_scsi_globals
again if it becomes necessary.

This patch removes the string from register component.
Removed are all instances in libs or hardcoded in apps.
Starting with this patch literal passed to register,
serves as name for the flag.
All instances of SPDK_LOG_* were replaced with just *
in lowercase.
No actual name change for flags occur in this patch.
Affected are SPDK_LOG_REGISTER_COMPONENT() and
SPDK_*LOG() macros.

SPDK iSCSI target didn't convert LUN ID from integer to structure
when it sends R2T PDUs. The next patch will fix the issue. Introducing
helper functions into SCSI library and using them will be clean. Hence
this patch adds two helper functions to convert LUN ID between structure
and integer.
The logic of helper functions is derived simply from the current
implementation in SPDK.

This was added a long time back for tracking an rte_mbuf
whose buffer was a different rte_mbuf - all related to
a userspace TCP stack that is no longer in development.
The concept isn't useful now, so remove it to reduce
the complexity of the tracing code.

Disambiguate the log components from the trace functionality
(include/spdk/trace.h).
The internal spdk_trace_flag structure and related functions will be
renamed in a later commit - this is just a find and replace on
SPDK_TRACE_* and SPDK_LOG_REGISTER_TRACE_FLAG().

None of these make sense as global parameters - they're
all only configurable per-disk. This is a large simplification
and removes the SCSI library's dependency on the config
file entirely.

This is far simpler, although it does limit the bdev
layer to unmapped just one range per command. In practice,
all of our code reports limits of just one range per command
anyway.
